  • Nausea.

Very Common (affecting more than 1 in 10 patients):

  • Diarrhea
  • Vomiting (typically resolves over time)
  • Constipation
    These effects are usually not severe and are most frequent when you first start tirzepatide; they tend to decrease as treatment continues. Constipation and vomiting are especially common when used for weight management, though they are also seen in type 2 diabetes treatment.

  • Low blood sugar when tirzepatide is used with both metformin and a sodium-glucose co-transporter 2 inhibitor, with symptoms similar to those described above.
  • Allergic reactions such as rash, itching, or eczema.
  • Dizziness, which is often observed in patients on weight management regimens.
  • Low blood pressure, seen in weight management.
  • Reduced appetite in patients with type 2 diabetes.
  • Abdominal pain, indigestion, bloating, burping, and flatulence.
  • Reflux or heartburn (GERD), caused by stomach acid rising into the esophagus.
  • Hair loss in some weight management cases.
  • Fatigue.
  • Injection site reactions, such as itching or redness.
  • A fast pulse, which is common in type 2 diabetes treatment and less so in weight management.
  • Mild increases in pancreatic enzymes (like lipase and amylase); however, an increase in amylase is uncommon in weight management.

  • Additional episodes of low blood sugar when tirzepatide is combined with metformin in type 2 diabetes, with symptoms similar to those above.

  • Raised calcitonin levels in the blood.

    • Norelgestromin: Cmax reduced by 55%, AUC by 23%, with a 4.5-hour delay in tmax.
    • Norgestimate: Cmax reduced by 66%, AUC by 20%, with a 2.5-hour delay in tmax.
  • These alterations are not considered clinically significant in women with a normal BMI, so no dose adjustment is required.

Special Considerations for Overweight or Obese Women

  • Limited data exists on the impact of tirzepatide on oral contraceptives in overweight or obese women, so reduced effectiveness cannot be ruled out.
