Mounjaro, the catchy brand name for tirzepatide, is a newly approved weekly injection for weight management.

Who can use Mounjaro?
In November 2023, the UK’s Medicines and Healthcare Products Regulatory Agency (or MHRA for short) licensed Mounjaro for use in adults with obesity (BMI of 30 or above) or those who are overweight (a BMI between 27 and 30) and have health conditions related to their weight [11].
On a side note, BMI may not be the best indicator for a healthy body; if you are a regular gym goer and pack a lot of muscle, you may get misdiagnosed as fat. Please don’t take it personally; BMI is trying its best.

How does Mounjaro work?
Mounjaro is an injectable medication that aids weight loss and controls blood sugar levels.

Does Mounjaro cause weight loss?

The Surmount 1 trial
This trial which was carried out over the course of 72 weeks, saw individuals lose up to 22.5% of their body weight when taking the Mounjaro 15mg injection [14].
The Surmount 4 trial
This trial dealt with weight loss in severely obese people, saw a massive 24.2% weight loss in those who took the Mounjaro 15mg injection [15].

What strengths of Mounjaro are available?

It is taken weekly as a subcutaneous injection, usually in the stomach or thigh or anywhere with a bit of fat.

What makes Mounjaro different from it’s competitors is that it’s the first medication to combine two gut hormones: GLP-1 (glucagon-like peptide-1) and GIP (glucose-dependent insulinotropic polypeptide) [12]. Â
These hormones also known as “incretins” increase insulin release when the food that has been consumed makes it’s way to the gastrointestinal tract. These hormones enable better appetite control and boosts your metabolism.
Because Mounjaro combines GLP-1 and GIP, it’s often called a “twincretin.” This double whammy not only controls your appetite and slows stomach emptying down but also enhances the weight-loss effects compared to if these hormones were working alone [12].
In short, Mounjaro’s allows you to eat less, stay satisfied, and lose more weight.

Can you take Mounjaro with levothyroxine?
There are no interactions between Mounjaro and levothyroxine.

However, just because there is no listed interactions between Mounjaro and levothyroxine, it doesn’t mean that you should not take precautionary measures, as the following case study demonstrates.

A case study of increased thyroid levels after taking semaglutide
A study in the Journal of the American Pharmacists Association highlights the importance of closely monitoring hormone levels while taking Mounjaro or a GLP-1 receptor agonist, like Wegovy.
The case study of a 43-year-old woman who had her thyroid taken out and was taking levothyroxine to treat hypothyroidism. After starting for weekly GLP-1 injection, semaglutide, she experienced significant suppression in her thyroid-stimulating hormone (TSH). This change meant her levothyroxine dose to be cut down by 25% [17].

Does semaglutide decrease TSH levels?
The exact reason behind the above patient’s fall in TSH levels we are not fully aware of, but this may be due to:
- A direct effect of GLP-1 on TSH levels.
- Delayed stomach emptying caused by the medication, which alters levothyroxine absorption.
- Weight loss associated with GLP-1 use (a whopping 13 kg loss in this case), can reduce the need for thyroid hormone replacement.
Other case studies found a similar effect when combining TSH when GLP-1 receptor agonists were combined [18].

Why should you be careful before taking Mounjaro with levothyroxine
The case study above demonstrated that GLP-1-containing medications, like Mounjaro, can interfere with other treatments, especially those which require precise dosing, such as levothyroxine.
So if you are taking levothyroxine to treat hypothyroidism can you still take Mounjaro? Yes, you can take the Mounjaro with levothyroxine, but you will need to have your thyroid levels checked more regularly.
