Paracetamol's central action

Paracetamol's analgesic action is primarily central. It is thought to inhibit a variant of the cyclo-oxygenase enzyme found in the brain and spinal cord, reducing prostaglandin synthesis in the central nervous system and raising the pain threshold. Unlike NSAIDs such as ibuprofen, paracetamol has minimal activity against peripheral inflammation, which is why it works well for pain but less well for inflammatory conditions like arthritis where peripheral prostaglandin activity is the main driver. It also has a well-established antipyretic effect, reducing elevated body temperature by acting on the hypothalamic heat-regulating centre.

Codeine's prodrug pathway

Codeine is a prodrug, meaning it is pharmacologically inactive in its original form and must be converted to an active metabolite to exert its effects. In the liver, the enzyme CYP2D6 converts approximately 5 to 10 per cent of a codeine dose into morphine, which then binds to mu-opioid receptors in the brain, spinal cord, and peripheral tissues. Mu-opioid receptor activation reduces the transmission of pain signals, alters the emotional response to pain, and produces mild sedation. The remaining codeine is metabolised to norcodeine and codeine-6-glucuronide, which contribute minor additional analgesic activity.

Why people respond differently

The degree to which codeine is converted to morphine varies significantly between individuals, depending on CYP2D6 genetic status. There are four broad phenotypes. Poor metabolisers, who carry two non-functional CYP2D6 alleles, produce very little morphine and may find the codeine component provides minimal benefit. Intermediate metabolisers have reduced enzyme activity. Extensive metabolisers, the majority of the population, metabolise codeine at a typical rate. Ultra-rapid metabolisers, who carry gene duplications leading to excessive CYP2D6 activity, convert codeine to morphine rapidly and in larger quantities, creating a risk of morphine toxicity even at standard doses.

Take the lowest effective dose

Take the lowest number of tablets that provides adequate relief. For many people, one tablet every six hours is sufficient; two tablets every four to six hours is the higher end of the adult dose range and should be reserved for when one tablet genuinely has not been enough. Swallow tablets with a full glass of water. Taking them after a light meal or snack reduces the likelihood of nausea, which is the most common short-term side effect. Space doses at least four hours apart to allow the paracetamol component to clear partially before the next dose.

Check every other medicine for paracetamol

Check every other medicine in your cabinet for paracetamol before starting a course of Co-Codamol 8/500mg. Cold and flu remedies, night-nurse products, and many other combination medicines contain paracetamol. Taking them alongside Co-Codamol 8/500mg can push your daily paracetamol intake dangerously high without you realising it. The safe daily limit for adults is 4000mg, which is eight standard paracetamol tablets. At two Co-Codamol 8/500mg tablets four times daily, you are already at that ceiling, leaving no room for additional paracetamol from other sources.

Three days, then stop

Keep to three consecutive days. This is the single most important instruction on the pack, and it doesn't change just because you have a larger 100-tablet supply at home. Who should not take this medicine

It is contraindicated in children under 12 years of age, in people known to be ultra-rapid metabolisers of codeine, in those with significant respiratory depression or obstructive airways disease, and in those with severe hepatic impairment. Codeine dependence

Codeine dependence is a recognised risk even with over-the-counter formulations. The risk is low within a three-day course for most people, but it increases with longer use, higher doses, and in individuals with a predisposition to substance dependence. Paracetamol liver toxicity is the most serious risk with this medicine, but it is almost entirely preventable. The maximum safe adult dose of paracetamol is 4000mg in 24 hours, equivalent to eight standard 500mg tablets. Exceeding this, even by a modest amount over several days, can cause serious and sometimes irreversible liver damage. The danger is that early symptoms of paracetamol overdose, nausea, vomiting, and abdominal pain, are mild and may not appear for 24 hours, during which significant liver injury may already be occurring. If you think you may have taken too much, seek emergency care immediately.

Pregnancy and breastfeeding

Codeine is not recommended during pregnancy, particularly in the third trimester. It is not safe for breastfeeding mothers, as codeine passes into breast milk and, in ultra-rapid metabolisers, can result in the infant receiving toxic morphine levels. Common side effects

CNS depressants

CNS depressants interact with the codeine component by additive or synergistic sedation and respiratory depression. This group includes alcohol, benzodiazepines, other opioid analgesics, sedating antihistamines (found in some sleep aids and allergy medicines), tricyclic antidepressants, and antipsychotics. Combining codeine with any of these increases the risk of excessive sedation, impaired driving ability, and, in severe cases, life-threatening respiratory depression. Avoid alcohol completely during a course of Co-Codamol 8/500mg.

MAOIs

Monoamine oxidase inhibitors (MAOIs) are contraindicated with codeine. MAOIs include certain antidepressants such as phenelzine and tranylcypromine, and some medicines used in Parkinson's disease. Codeine should not be taken during MAOI treatment or within 14 days of stopping an MAOI. If you are unsure whether any of your medicines are MAOIs, ask your pharmacist.

Anticoagulants and CYP2D6 inhibitors

Warfarin and anticoagulants may be affected by regular high-dose paracetamol.
