EURneffy 2mg nasal spray is a ready-to-use, single-dose adrenaline (epinephrine) nasal spray for the emergency treatment of anaphylaxis. Each device delivers 2mg adrenaline in 100 microlitres of a clear solution (colourless to pink-brownish).
You use EURneffy by placing the nozzle into one nostril and pressing the plunger. The device releases the full dose in one spray. There’s no priming step, and you don’t need to inhale. So, it’s designed to be simple to use in a high-stress emergency.
. The MHRA approved EURneffy on 18 July 2025, which made it the first needle-free adrenaline option for anaphylaxis available in the UK. It’s now available on prescription.
For many people, a needle-free format can remove a real barrier to giving adrenaline quickly. EURneffy also offers a longer shelf life than many auto-injectors. It has a 30-month shelf life and can tolerate temperature excursions up to 50°C. By comparison, many injectable devices have a shorter shelf life and lower temperature limits.
However, EURneffy doesn’t automatically replace your auto-injector. Your allergy specialist should decide the safest plan for you. In other words, some people may carry both, or carry EURneffy alongside an EpiPen or Jext.
A lower-dose 1mg version for children weighing 15–30kg is approved in the US and is under review in Europe. So, UK availability for that dose may follow, but timing can vary.

Treatment dosage EURneffy 2mg nasal spray

The recommended dose is a single nasal administration of 2mg of adrenaline. At the first sign of anaphylaxis, insert the nozzle into one nostril until your fingers touch your nose and press the plunger firmly until it snaps up. The device delivers its full 2mg dose in one spray.

Don’t sniff during or after administration, since sniffing may reduce absorption. After using the spray, dial 999 immediately, regardless of whether you feel better. Anaphylaxis can rebound or progress unpredictably, and you need to be monitored by emergency services.

If symptoms continue, return, or worsen after 5 minutes (or if there has been any error in administering the first dose), use a second EURneffy nasal spray in the same nostril as the first.

The active ingredient is adrenaline (epinephrine), 2mg per single-dose container. Adrenaline is a hormone produced naturally by the adrenal glands in response to stress, and it's the body's own emergency response chemical. When given exogenously during anaphylaxis, adrenaline acts on alpha-adrenergic receptors to raise blood pressure and reduce swelling, and on beta-adrenergic receptors to open the airways, increase heart rate and reduce the release of further inflammatory mediators from mast cells . The combined effect can reverse the cardiovascular collapse and airway obstruction of anaphylaxis within minutes.

EURneffy's formulation also contains benzalkonium chloride (40 micrograms per dose), sodium metabisulfite (5 micrograms), dodecyl maltoside (a permeation enhancer that helps the adrenaline cross the nasal lining quickly), edetate disodium, hydrochloric acid, and water for injections. How to use EURneffy 2mg nasal spray

At the first sign of anaphylaxis, don't wait to be sure. Use EURneffy. Hold the device with your thumb on the base and two fingers either side of the nozzle. Insert the nozzle into one nostril until your fingers touch the outside of your nose. Hold the device straight into the nose, pointed slightly toward your forehead, not angled toward the inner wall (the septum) or the outer wall of the nostril. Press the plunger firmly until it snaps up. You'll feel the spray release. Don't sniff during or after.

Position yourself to support your circulation. Lie flat with your feet slightly raised, unless you're struggling to breathe, in which case sit up slightly while still leaning back. Pregnant women should lie on the left side to take pressure off the major blood vessels. Don't stand up. Don't walk. Call 999 immediately or have someone with you call. Tell the operator you're using adrenaline for anaphylaxis. Stay where you are until paramedics arrive, even if symptoms appear to be settling.

Practical tips from our pharmacists: practise the technique with a demo device (your prescriber can arrange one) before you need it for real; carry two devices everywhere, since one may not be enough; tell people close to you (family, friends, colleagues, school staff) how to use the spray and where you keep it; replace devices before their expiry date even if unused; and never store EURneffy in a freezer, since a frozen device will not deliver adrenaline (allow at least one hour to thaw at room temperature if accidentally frozen, and discard if the contents look frozen or partly frozen). Warnings and precautions for EURneffy 2mg nasal spray

EURneffy has no listed contraindications, because in the context of a life-threatening allergic reaction, the benefit of giving adrenaline almost always outweighs the theoretical risks. However, several precautions apply. People with cardiovascular disease (ischaemic heart disease, coronary artery disease, arrhythmias, severe hypertension, cardiomyopathy) are more vulnerable to adrenaline-related side effects such as raised blood pressure and arrhythmias. This doesn't stop them from using EURneffy in an emergency, but it makes the conversation with a specialist about whether nasal or intramuscular adrenaline is the better fit more important.

Hyperthyroidism, Parkinson's disease, diabetes, renal impairment, and low blood potassium can all be temporarily worsened by adrenaline, so people with these conditions should be carefully assessed. People with significant structural or anatomical nasal problems (severe deviated septum, post-surgical changes, large polyps, recent nasal trauma) may have unpredictable absorption from a nasal spray, and an auto-injector may be the safer first-line in that situation. Tell your prescriber about any of these factors during the consultation.

Patient training matters more for EURneffy than for many medicines. Because anaphylaxis is rare for individual patients but high-stakes when it happens, regular practice with a demo device is genuinely important. Studies show that even experienced users of auto-injectors make technique errors under pressure; a needle-free device removes one source of error but doesn't eliminate the need to practise. Pregnancy and breastfeeding are usually not barriers to using EURneffy when needed. There's no clinical data specifically in pregnant women, but adrenaline has been used in pregnancy for anaphylaxis for decades, and the consensus is that the risk of untreated anaphylaxis to mother and baby far outweighs the theoretical risk of the medicine. EURneffy can be used in breastfeeding women because adrenaline has poor oral bioavailability and a very short half-life, so transfer to the infant via breast milk is minimal.

A common misunderstanding: EURneffy doesn't replace your existing auto-injector unless your specialist explicitly switches you over. For many people with anaphylaxis risk, the right plan is to carry one EURneffy alongside one auto-injector, giving you a needle-free option you'd reach for first and a backup if needed. Side effects of EURneffy 2mg nasal spray

The most frequently reported adverse reactions in clinical trials of EURneffy 2mg (only after a second 4mg total dose) included throat irritation (18.8%), headache (17.6%), nasal discomfort (12.9%) and feeling jittery (10.6%). None of these were considered serious. These are exactly the side effects you would expect from giving a strong dose of adrenaline, regardless of the route, since adrenaline is by nature a stimulant.

Common (occurring in 1-10% of users) side effects after a single 2mg dose include anxiety, tremor, palpitations, runny nose (rhinorrhoea), nasal swelling (nasal oedema), nasal pain (rhinalgia), nasal congestion, increased blood pressure, and increased heart rate. Most of these are short-lived and reflect adrenaline's expected pharmacology rather than a hypersensitivity to the medicine itself.

Less common and rare effects can include chest discomfort, sweating, nausea or vomiting, weakness, dizziness, pale skin (pallor), and feelings of agitation or impending doom (a documented effect of adrenaline that doesn't reflect a real threat). Very rarely, serious adrenergic effects can occur, including ventricular arrhythmias in people with underlying heart disease, hypertensive crisis, or cerebral haemorrhage from a sharp rise in blood pressure. These are most likely when adrenaline is given to someone with significant pre-existing cardiovascular disease.

Overdose is unlikely with a single 2mg dose but possible if multiple doses are given in succession without medical supervision. Symptoms can include severe headache, chest pain, dizziness, nausea, blurred vision, and significant rises in blood pressure. If you have used more than two doses (4mg) of EURneffy outside of a hospital setting, call 999 and inform the emergency services. Continued monitoring will usually be necessary.

Suspected side effects can and should be reported to the MHRA via the Yellow Card scheme at yellowcard.mhra.gov.uk. EURneffy is a recently approved product, so post-marketing surveillance is particularly important; reporting helps build the safety picture for everyone.

Drug interactions with EURneffy 2mg nasal spray

Several medicines can interact with adrenaline. Tricyclic antidepressants (amitriptyline, nortriptyline), monoamine oxidase inhibitors, levothyroxine, certain antihistamines, and some Parkinson's medicines (levodopa) can intensify the effects of adrenaline. In emergency anaphylaxis treatment this rarely changes the decision to give adrenaline, but it can mean side effects are more pronounced.

Beta-blockers (propranolol, bisoprolol, metoprolol, atenolol) can blunt the response to adrenaline, since they block the beta-adrenergic receptors that adrenaline acts on. People on beta-blockers may need a second dose more often, and they may also be more prone to alpha-adrenergic side effects like raised blood pressure once the beta-blockade has been overcome. If you're on a beta-blocker for cardiovascular disease, mention this clearly during the consultation; sometimes the medicine can be reviewed and switched to something with less impact on adrenaline response.

Diuretics and other medicines that lower potassium can be exacerbated by adrenaline, which itself can lower potassium briefly. This rarely matters acutely but can be relevant in people with cardiac conditions. ACE inhibitors and angiotensin receptor blockers don't directly interact with adrenaline but can complicate anaphylaxis treatment because they affect the blood pressure regulation systems the body would normally use to compensate.

Other nasal sprays applied around the same time can affect EURneffy's absorption, although in a real anaphylaxis emergency the nasal spray takes priority and any concurrent nasal medication is irrelevant. Diabetic patients on insulin or oral hypoglycaemics may need to monitor blood glucose after using adrenaline, since adrenaline raises blood glucose temporarily. None of these interactions are reasons to avoid EURneffy in anaphylaxis; they're reasons to discuss the bigger medication picture during your consultation.

Alcohol doesn't pharmacokinetically interact with adrenaline, but heavy intoxication can complicate any emergency medical situation. There are no relevant interactions with the contraceptive pill, statins, common antibiotics, or most other regular medications.
