Low testosterone is common, treatable, and worth getting right
No templates, no guesswork. We read your bloods, listen to your symptoms and build a plan around them — the BSSM way, with two morning samples before any diagnosis, all by post.
Dispatch Manifest · TRT Pathway
Two morning samples, by post
Sample one — home TRT kit
Finger-prick, taken 07:00–11:00. Testosterone, free testosterone, SHBG, FAI, albumin.
Sample two — full TRT panel
A second morning sample on a separate day. Confirms the result and screens your wider health.
Clinician review
A prescriber interprets both samples against BSSM action levels and your symptoms.
Meet Dr Ada
Hi, I’m Dr Ada Jex Cori
Here’s how I see it: no two men are the same, so no two plans should be either. We don’t reach for a template — we run one full system check, read what your body is actually telling us, and shape the treatment around it.
That’s the whole idea behind Courier. Treatment that fits you — your bloods, your symptoms, your pace. Most of it happens at home, by post, on your terms.
Dr Ada is A Fictional Brand Ambassador.
Your actual care is prescribed & signed off by UK-registered clinicians.

Our ethos
“Treatment that fits you” — what we mean by it
It’s not a slogan we bolt on at the end. It’s the order we do things in, and the things we refuse to skip.

Honest
If testosterone isn’t the cause, we’ll say so — and point you somewhere more useful.
What it is
When the body makes too little testosterone
Testosterone shapes energy, mood, muscle, bone, libido and erections. Levels fall gradually with age, but they can also drop for medical reasons at any age. When low levels line up with real symptoms, it’s called testosterone deficiency — and it’s one of the more under-recognised conditions in men’s health.
Sexual
The clearest signals
Lower sex drive
Weaker or less frequent erections
Loss of early-morning erections
Reduced ejaculation
Physical
How the body feels
Persistent tiredness and low energy
Loss of strength or muscle
More belly fat, less stamina
Poor sleep
Mood & mind
The quieter symptoms
Low mood or irritability
Reduced motivation and focus
Less enjoyment of life
Brain fog at work
More likely if you have:
Many of these symptoms have causes other than testosterone — which is exactly why a screening check and two confirmed blood samples come before any treatment.
Step 1 · Self-assessment
The ADAM symptom check
The Androgen Deficiency in the Ageing Male (ADAM) questionnaire is a validated 10-question screen used worldwide. Answer honestly about how you’ve felt recently — it takes about two minutes and nothing is stored.

Ten quick questions
This is a screening tool, not a diagnosis. A positive screen means it’s worth investigating with a blood test — it does not confirm low testosterone, and a negative screen doesn’t rule it out.
Step by step
How you get TRT with Courier Pharmacy
The British Society for Sexual Medicine (BSSM) is clear: testosterone should be measured on two separate morning samples before a diagnosis is made. Our pathway is built around that standard.
01
Symptom check
Complete the ADAM questionnaire above. A positive screen points you toward testing.
02
First morning sample
The home TRT kit, taken between 07:00–11:00. A simple finger-prick: testosterone, free testosterone, SHBG, FAI and albumin.
03
Confirmatory sample
A second morning sample on a different day, using the full Courier Pharmacy TRT panel — this confirms the result and checks your wider health.
04
Clinician review
A prescriber reads both samples against BSSM action levels and your symptoms, and checks for anything that would make TRT unsafe.
05
Treatment & monitoring
If TRT is appropriate, treatment begins with planned follow-up blood tests to keep it safe over time.
What the prescriber is looking for
BSSM action levels (interpreted alongside your symptoms)
Total testosterone below 8 nmol/L usually supports treatment
8–12 nmol/L is borderline — free testosterone is calculated to decide
A higher threshold (~14 nmol/L) may apply if you have pre-diabetes
Total testosterone above 12 nmol/L usually doesn’t need treatment
Where testosterone is low, LH, FSH and prolactin help find the cause
Baseline haematocrit & PSA are checked before and during treatment
The two samples
What’s in each blood test
Every pathway starts with the same two morning samples. The first confirms your testosterone simply and quickly; the second gives the prescriber the fuller picture needed to start treatment safely.

TRT Options
One condition, several treatment routes — matched to you
Testosterone can be given by injection or applied to the skin, with fertility-preserving and symptom-support medicines used where appropriate. Which medicine, which form and what dose are agreed with your prescriber at your assessment — never picked from a shelf.
Treatment routes
How testosterone gets into your system
Testosterone can’t be taken reliably as an everyday tablet — so TRT is given in ways that let it into the bloodstream steadily and safely. Each route has its own rhythm and trade-offs. There’s rarely one “best” for everyone; there’s a best for you, and your prescriber works that out with you.
Under the skin
Gold standard
Subcutaneous injection
A small injection into the fatty layer just beneath the skin, usually weekly or twice-weekly. The route many clinicians now prefer first.
Pros
Steady, stable levels — fewer swings in energy, mood and libido
Precise dose that’s easy to fine-tune
Small, fine needle — gentler than into muscle
Simple to self-administer at home once shown
Cons
Regular injections — not for the needle-averse
Occasional minor injection-site reactions
Needs red-cell (haematocrit) monitoring, as all TRT does
Into the muscle
Intramuscular injection
An injection into the muscle — given either as shorter-acting doses or as a long-acting depot every few months.
Pros
Long-established and effective
Long-acting forms need only a few injections a year
Shorter-acting forms are often lower cost
Cons
Levels can peak then dip between shorter-acting doses
Long-acting forms are slow to adjust if the dose isn’t right
Long-acting forms are slow to adjust if the dose isn’t right
Applied to the skin
Skin gel
A gel applied to the skin once a day — needle-free, with steady day-to-day levels.
Pros
No needles
Steady daily levels
Easy to pause or adjust
Cons
Must be applied every single day
Can rub off onto others through skin contact — must be covered and hands washed, avoiding contact with partners and children until absorbed
Absorption varies; can irritate the skin
Less commonly used
Other routes
Skin patches, long-acting implants placed under the skin, and newer oral forms also exist. Your prescriber will only raise these if they genuinely suit you — they each carry their own routines and trade-offs.
Protecting fertility
Fertility-preserving options
Testosterone on its own can lower sperm production — so if you may want children, now or later, fertility-preserving medicines can run alongside your treatment (or sometimes in place of it) to keep your body’s own production ticking over. They come in two simple forms, and your prescriber decides whether and which to use:

Compounding at Courier
When the standard option isn’t the right fit, we can make one to order
Courier is a compounding pharmacy. Where a prescriber decides it’s appropriate for you, we can prepare bespoke, made-to-order preparations — tailored to the individual, under full clinical governance. It’s the same principle that runs through everything we do.
Good to know
Common questions
Testosterone naturally rises and falls through the day and varies from one day to the next, so a single reading can be misleading. The BSSM recommends two separate samples, both taken between 07:00 and 11:00, before a diagnosis is made. The first is a simple home finger-prick; the second is a fuller panel that also screens your wider health.
No. ADAM is a screening questionnaire, not a diagnosis. It’s good at flagging men who should be tested, but its symptoms overlap with many other conditions — stress, poor sleep, thyroid problems and more. Only a blood test, read by a prescriber alongside your symptoms, can confirm testosterone deficiency.
Testosterone therapy can reduce sperm production. If you may want children, tell the prescriber — fertility-preserving medication such as hCG is included in the Silver and Gold packages and can be used alongside treatment where appropriate. This is something the prescriber will discuss with you directly.
After starting, you’ll have follow-up blood tests at one month, six months and twelve months, then yearly. These check that your testosterone is in range and keep an eye on safety markers such as haematocrit (red-cell concentration) and PSA. Gold adds two further monitoring tests for closer oversight.
The DUTCH test uses dried urine to map how your body processes hormones and their breakdown products over a day — detail that a single blood draw can’t show. It’s an optional, advanced add-on included in the Gold package for men who want the fullest hormonal picture.
No. TRT is a prescription-only treatment. It’s offered only after two confirmed low morning samples, alongside genuine symptoms, and once a prescriber has checked there’s nothing that would make it unsafe for you. If testosterone isn’t the cause of your symptoms, we’ll tell you and point you in the right direction.
