How to take naltrexone 25mg

Standard TSM dosing

The core Sinclair Method protocol:

  • Take ONE 25mg capsule approximately ONE HOUR before drinking alcohol
  • Only take on days you plan to drink alcohol
  • Do NOT take on non-drinking days
  • Swallow the capsule whole with water
  • With or without food (small snack helps if nausea occurs)
  • Wait the full hour before consuming alcohol
  • Drink as you normally would

The 50mg step-up option

If your prescriber recommends stepping up:

  • Take TWO 25mg capsules together = 50mg total
  • Still take ONE HOUR before drinking
  • Both capsules together, not spread out
  • Only on drinking days
  • Same one-hour timing rule applies

  • Plan drinking occasions about an hour ahead
  • Set a phone reminder for the pre-drinking dose

  • Severe physical alcohol dependence requiring detox
