Description

Product description Tadalafil 20mg Tablets

What tadalafil 20mg tablets are

Tadalafil is a selective phosphodiesterase type 5 (PDE5) inhibitor, developed in the 1990s and licensed in the UK as Cialis in 2003. So the branded Cialis 20mg patent expired in 2017, since when generic tadalafil 20mg tablets have been widely available. In particular, tadalafil 20mg tablets are a UK-licensed prescription-only medicine (POM), supplied as generic and multiple branded forms. Furthermore, one film-coated tablet contains 20mg of tadalafil, the top on-demand strength.

Where tadalafil sits in erectile dysfunction treatment

Current UK guidance (NICE CKS Erectile Dysfunction and BSSM 2017) places PDE5 inhibitors as first-line treatment for ED in men who have no contraindication. So the choice between sildenafil, tadalafil, vardenafil, and avanafil depends on how often sex is anticipated, how much flexibility the patient wants, and tolerance of side effects.

In particular, tadalafil’s long half-life (around 17.5 hours) and 36-hour window of action give it a different patient fit from the shorter-acting PDE5 inhibitors. Furthermore, tadalafil is also available in a 2.5mg and 5mg daily low-dose form for men who prefer continuous rather than on-demand use, or for the combined treatment of ED and benign prostatic hyperplasia (BPH).

Why tadalafil’s long duration matters

Tadalafil has an elimination half-life of around 17.5 hours, compared with 3 to 5 hours for sildenafil and vardenafil. So its clinical effect lasts up to 36 hours after a single on-demand dose. In particular, this detaches dosing from sexual timing, which is the main reason patients and partners often prefer it. Furthermore, this is where the “weekend tablet” nickname comes from, since a single Friday evening dose can cover Saturday as well.

Not an aphrodisiac, not a cure

Tadalafil restores the physiological response to sexual stimulation. So it does not create arousal, does not fix emotional or relationship-driven ED, and does not increase libido. In particular, this is why we always ask about the context of ED during the consultation. Furthermore, if the ED is not working with tadalafil, that often points to a non-vascular cause (psychological, hormonal, relationship, medicines) that needs looking at separately.

Tadalafil 20mg Tablets

Tadalafil 20mg tablets, the full-strength on-demand PDE5 inhibitor for erectile dysfunction

Tadalafil 20mg tablets are a licensed prescription-only PDE5 inhibitor used on demand to treat erectile dysfunction (ED) in adult men. So this is the full-strength on-demand dose of tadalafil, the active ingredient also sold under the brand name Cialis. In particular, tadalafil is distinct from sildenafil (Viagra) because of its much longer duration of action, which is why it is sometimes called the “weekend tablet” and why it fits patients who want flexibility rather than a tight time window. Furthermore, at Courier Pharmacy we supply tadalafil 20mg tablets after a proper prescriber assessment, with clear advice on cardiovascular safety, nitrate interactions, and the realistic expectations around what PDE5 inhibitors can and cannot do.

When to see a doctor before starting or continuing tadalafil 20mg

Tadalafil is well tolerated for most patients. However, some situations need medical attention before starting, and any of these during treatment warrants contacting your prescriber:

  • Taking any form of nitrate (GTN spray or tablets, isosorbide mononitrate or dinitrate, or recreational “poppers” / amyl nitrite).
  • Taking riociguat (a soluble guanylate cyclase stimulator used in pulmonary hypertension).
  • Any history of heart attack in the last 90 days or stroke in the last 6 months.
  • Unstable angina or angina during sexual activity.
  • Uncontrolled high or low blood pressure.
  • Severe heart failure (NYHA class 3 to 4).
  • Serious uncontrolled arrhythmia.
  • Severe liver impairment.
  • End-stage kidney disease or dialysis.
  • Previous non-arteritic anterior ischaemic optic neuropathy (NAION) with any PDE5 inhibitor.
  • Hereditary degenerative retinal conditions (retinitis pigmentosa).
  • Priapism (an erection lasting more than 4 hours) is a medical emergency, go to A&E.
  • Sudden hearing loss, ringing in the ears, or dizziness during or after a dose.
  • New chest pain during or after sexual activity, or any symptoms of a possible heart attack.

For life-threatening symptoms (chest pain, stroke-like features, prolonged erection, sudden visual or hearing loss), call 999. For urgent but non-emergency concerns, call NHS 111.

Five key takeaways about tadalafil 20mg tablets

  • Licensed UK prescription-only medicine (POM), a phosphodiesterase type 5 (PDE5) inhibitor.
  • The full-strength on-demand dose of tadalafil for erectile dysfunction in men aged 18 and over.
  • Taken 30 minutes to 36 hours before sexual activity, maximum one tablet in any 24-hour period.
  • Absolutely contraindicated with any form of nitrate and with riociguat.
  • Not an aphrodisiac, so sexual stimulation is still needed for an erection to happen.

What is erectile dysfunction: overview

  • Erectile dysfunction (ED) is the persistent inability to get or keep an erection sufficient for satisfactory sexual activity.
  • It affects around 50 percent of men aged 40 to 70 to some degree, rising with age.
  • ED is often an early marker of cardiovascular disease, particularly in men under 60, because the penile arteries are smaller than the coronary arteries.
  • Common contributors include cardiovascular disease, diabetes, hypertension, obesity, smoking, sedentary lifestyle, psychological factors, and prescribed medicines.
  • PDE5 inhibitors like tadalafil are first-line pharmacological treatment in the UK (NICE CKS, BSSM 2017).

A brief history of tadalafil

Tadalafil was developed by Icos Corporation (later Lilly Icos) in the 1990s and licensed as Cialis in Europe and the US in 2003. So it was the third PDE5 inhibitor to reach the market after sildenafil (Viagra, 1998) and vardenafil (Levitra, 2003). In particular, its selling point from the start was its long duration of action, giving a more natural dosing-sex separation than sildenafil could offer. Furthermore, generic tadalafil became available after the European patent expired in 2017, substantially reducing the cost.

How tadalafil differs from sildenafil, vardenafil, and avanafil

All four UK-licensed PDE5 inhibitors share the same core mechanism. However, they differ in onset, duration, food sensitivity, and side-effect profile. So sildenafil (Viagra) acts within 30 to 60 minutes with 4 to 6 hours of effect; vardenafil (Levitra) is similar; avanafil (Spedra) is the fastest-acting at 15 to 30 minutes with 6 hours of effect; and tadalafil (Cialis) has the longest window at up to 36 hours. In particular, sildenafil absorption is significantly reduced by a heavy fatty meal, while tadalafil and avanafil are not. Furthermore, this is why the choice of PDE5 inhibitor is individualised to the patient’s lifestyle, sexual pattern, and preferences.

Why ED should be assessed, not just treated

Erectile dysfunction is often the first visible sign of early cardiovascular disease, especially in men under 60. So the arteries of the penis are smaller than the coronary arteries, which means endothelial dysfunction tends to show up as ED a few years before it shows up as angina or heart attack. In particular, this is why BSSM 2017 and NICE CKS both recommend a cardiovascular risk assessment and basic blood tests (lipids, HbA1c, testosterone) in men presenting with ED. Furthermore, treating only the ED without looking at the wider picture is a missed opportunity for prevention.

Why choose Courier Pharmacy for your tadalafil 20mg tablets

Registered, independent, and patient-led

Courier Pharmacy is a UK-registered independent pharmacy regulated by the General Pharmaceutical Council (GPhC 9010051). So when you order tadalafil through us, you work with a proper pharmacy team, not a warehouse. Furthermore, our independence means we can spend time on sexual health questions where a chain would not.

Specialist prescriber expertise

Our UK prescribers understand ED within its wider endocrine and metabolic context: testosterone, diabetes, hypertension, cardiovascular risk, and the medicines that cause or worsen it. In particular, this matters because ED is often one piece of a bigger health picture, and treating just the visible symptom misses the chance to address the drivers.

BSSM-aligned consultation

Our ED consultation is aligned with British Society for Sexual Medicine (BSSM) 2017 guidance. So we check cardiovascular risk, nitrate use, and the common secondary causes before dispensing. In particular, we routinely suggest baseline bloods (lipids, HbA1c, testosterone) for men who have not had them recently, and we flag when ED points to a wider workup that goes beyond a tablet.

Dr Ada Jex-Cori, our brand pharmacist

Our brand pharmacist is Dr Ada Jex-Cori. She represents the spirit of what we do. Named for Ada Lovelace, Sophia Jex-Blake, and Gerty Cori, she stands for evidence-led, unafraid-of-questions medicine. Her message runs through everything we prescribe:

You’re not broken. The system is. We’re here to change that.

For ED patients in particular, this matters. So many men wait years before seeking help because of embarrassment or because they assume ED is “just ageing” rather than a treatable medical condition that is also a cardiovascular risk marker. Furthermore, we treat ED as the real medical condition it is, not as a cosmetic or lifestyle request.

Care that keeps showing up

ED treatment is a long-term project. So we are here for the follow-up questions along the way: dose adjustments, switching between PDE5 inhibitors, when to think about daily low-dose rather than on-demand, and how ED treatment fits with other medicines you take. In particular, if your response to tadalafil is not what you hoped, we would rather help you troubleshoot than just send a repeat prescription.

Buy Tadalafil 20mg Tablets (Prescription Only) from Courier Pharmacy

Tadalafil 20mg is a prescription-only medicine (POM) supplied through our erectile dysfunction and men’s health consultation pathway at courierpharmacy.co.uk. So supply always follows a clinical assessment. In particular, new patients complete an ED consultation covering duration and severity of ED, cardiovascular history, current medicines, nitrate use, and lifestyle. However, established patients on stable tadalafil can complete a shorter reassessment.

The four steps

  • Complete a quick online consultation covering your ED pattern, cardiovascular history, current medicines, nitrate use, and lifestyle.
  • A UK prescriber (our Superintendent Pharmacist Shazlee Ahsan or a partner prescriber) reviews your answers.
  • If tadalafil 20mg is clinically appropriate, a prescription is issued.
  • We dispense and deliver discreetly to your door.

If tadalafil 20mg isn’t right for you

If our assessment shows a different approach fits your situation better, we’ll explain why and suggest the next best option. So this might mean:

  • Tadalafil 10mg on-demand if you want to start at a lower dose or have mild-to-moderate kidney or liver impairment.
  • Tadalafil 2.5mg or 5mg daily if you want continuous coverage or also have BPH symptoms.
  • Sildenafil 50mg or 100mg if you prefer a shorter-acting PDE5 inhibitor (now OTC as Viagra Connect 50mg from a pharmacy consultation).
  • Vardenafil (Levitra) if sildenafil has not worked and you prefer a shorter-acting option than tadalafil.
  • Avanafil (Spedra) if a very fast onset (15 to 30 minutes) matters most.
  • A GP or men’s health clinic referral if cardiovascular risk needs assessing first.
  • A baseline bloods pack (lipids, HbA1c, testosterone) through your GP before starting, if you have not had these recently.
  • A testosterone replacement discussion if blood tests confirm testosterone deficiency.
  • Lifestyle support (weight loss, smoking cessation, cardiovascular fitness) if these are modifiable contributors.
  • Non-PDE5 options (vacuum pump, intracavernosal injection, penile prosthesis) via urology if PDE5 inhibitors have not worked.

Above all, we won’t dispense a medicine that isn’t right for your situation. In short, honest guidance matters more than making the sale.

Active ingredient in tadalafil 20mg tablets

Tadalafil

Each film-coated tablet contains 20mg of tadalafil. So tadalafil is the sole active ingredient. In particular, tadalafil is a selective phosphodiesterase type 5 (PDE5) inhibitor. Furthermore, its selectivity for PDE5 over PDE6 and PDE11 is similar to that of sildenafil but with much slower clearance, which is what gives tadalafil its distinctive long duration of action.

Other ingredients

The exact excipient list depends on the specific brand or generic dispensed. So typical tablet-core excipients include lactose monohydrate, croscarmellose sodium, hydroxypropyl cellulose, microcrystalline cellulose, sodium laurilsulfate, and magnesium stearate, with a film coating containing hypromellose, talc, triacetin, titanium dioxide, and iron oxide colourings. Furthermore, if you have a lactose intolerance or a specific excipient allergy, tell our team before ordering so we can confirm the excipients of the brand being dispensed.

What are tadalafil 20mg tablets for?

Tadalafil 20mg tablets are licensed for the on-demand treatment of erectile dysfunction in adult men. So the licensed indication is specific:

Erectile dysfunction (ED) in adult men

The main use is to treat erectile dysfunction in men aged 18 and over, taken as an on-demand single dose before anticipated sexual activity. So this covers vasculogenic, neurogenic, mixed, and some psychogenic ED. In particular, response rates in trials are around 70 to 80 percent of men achieving successful intercourse at 20mg, compared with around 30 to 40 percent on placebo.

ED associated with diabetes

Tadalafil is effective in ED associated with diabetes, though response rates are slightly lower than in non-diabetic patients. So many diabetic men respond well to 20mg on-demand. In particular, if 20mg does not give adequate response, the daily low-dose (5mg) option is sometimes more effective in this group.

ED after prostatectomy or pelvic surgery

Tadalafil can help ED following radical prostatectomy or pelvic surgery, though results are better when the surgery has been nerve-sparing. So many urologists recommend regular PDE5 inhibitor use (penile rehabilitation) in the first months after surgery. In particular, this is sometimes done with daily low-dose tadalafil rather than on-demand 20mg.

Benign prostatic hyperplasia (BPH), with the daily low-dose only

Tadalafil at 5mg once daily is licensed for the symptoms of benign prostatic hyperplasia (BPH), alone or combined with ED treatment. However, the 20mg tablet is not licensed for BPH; the 5mg daily dose is. So if BPH is also an issue, tadalafil 5mg daily may fit better than this on-demand 20mg tablet.

Where tadalafil 20mg is not the right choice

However, some situations need a different approach:

  • Any current use of nitrates (GTN spray, isosorbide mononitrate, isosorbide dinitrate) or amyl nitrite ("poppers").
  • Current use of riociguat (used in pulmonary hypertension).
  • Heart attack within the last 90 days or stroke within the last 6 months.
  • Unstable angina or angina during sexual activity.
  • Severe heart failure (NYHA class 3 to 4).
  • Severe uncontrolled hypertension or hypotension.
  • Severe hepatic impairment.
  • End-stage kidney disease (dialysis).
  • Previous non-arteritic anterior ischaemic optic neuropathy (NAION) with any PDE5 inhibitor.
  • Hereditary degenerative retinal conditions (retinitis pigmentosa).
  • Women (not licensed for female sexual dysfunction).
  • Under 18 (not studied).

How do tadalafil 20mg tablets work?

Tadalafil works through several linked steps centred on nitric oxide and cyclic GMP signalling in the smooth muscle of the penis. So its effect comes from amplifying the natural signal that produces an erection rather than creating one on its own. In particular, six aspects of the mechanism are worth understanding.

The normal erection pathway

When a man is sexually stimulated, nerve endings in the penis release nitric oxide (NO). So NO activates the enzyme guanylate cyclase in smooth muscle cells of the corpora cavernosa, which produces cyclic guanosine monophosphate (cGMP). In particular, cGMP relaxes the smooth muscle, allowing arterial blood flow in and compressing venous outflow, which produces an erection. Furthermore, phosphodiesterase type 5 (PDE5) is the enzyme that breaks cGMP down, ending the erection.

Selective PDE5 inhibition

Tadalafil binds PDE5 selectively, so it stops cGMP being broken down. So cGMP levels stay elevated for longer in response to the normal sexual stimulus. In particular, this prolongs smooth muscle relaxation and makes a satisfactory erection more likely. Furthermore, no sexual stimulus means no NO, no cGMP, and no erection; tadalafil amplifies the pathway but does not start it.

The 36-hour window of action

Tadalafil has an elimination half-life of around 17.5 hours, far longer than sildenafil (3 to 5 hours) or vardenafil (4 to 5 hours). So its effect persists for up to 36 hours after a single dose. In particular, this does not mean a continuous 36-hour erection; it means that during that 36-hour window, sexual stimulation is more reliably followed by a satisfactory erection. Furthermore, this long window is why tadalafil is sometimes called the "weekend tablet".

Absorption, metabolism, and elimination

Tadalafil is absorbed from the gut regardless of food, with peak plasma levels around 2 hours after an oral dose. So heavy fatty meals do not significantly affect its absorption, unlike sildenafil. In particular, tadalafil is metabolised in the liver by CYP3A4, which is why strong CYP3A4 inhibitors (ketoconazole, ritonavir, clarithromycin) raise tadalafil levels. Furthermore, tadalafil and its metabolites are excreted mainly in faeces and partly in urine.

Why tadalafil can affect vision and hearing

PDE5 has close cousins (PDE6 in the retina and PDE11 in various tissues). So tadalafil has some activity on PDE11 and small residual activity on PDE6. In particular, this explains the rare visual effects (blue-tinge vision is more common with sildenafil, which inhibits PDE6 more; tadalafil tends to cause less of this but has been linked to the rare NAION cases). Furthermore, sudden hearing loss is a rare class effect of PDE5 inhibitors that needs immediate attention if it occurs.

Why tadalafil interacts with nitrates

Nitrates (GTN, isosorbide mononitrate, amyl nitrite) release nitric oxide, which raises cGMP throughout the vasculature. So combining a nitrate with a PDE5 inhibitor (which stops cGMP being broken down) produces a massive, prolonged drop in blood pressure that can be fatal. In particular, this is why nitrates are an absolute contraindication, including recreational "poppers" that some men use socially. Furthermore, the nitrate washout after stopping tadalafil needs at least 48 hours before a nitrate can be given safely (and longer for the daily low-dose form).

How to take tadalafil 20mg tablets

The basic pattern

  • Adult men 18 and over: one 20mg tablet, 30 minutes to 36 hours before anticipated sexual activity.
  • Maximum one tablet in any 24-hour period.
  • Swallow the tablet whole with a glass of water.
  • Can be taken with or without food.
  • Sexual stimulation is still needed for an erection to happen.

Food, alcohol, and grapefruit

Tadalafil is not significantly affected by food, including fatty meals. However, heavy alcohol can impair erectile function in its own right and may add to tadalafil's mild blood-pressure-lowering effect, so moderation is sensible. In particular, grapefruit and grapefruit juice inhibit CYP3A4 and can raise tadalafil levels, so limit grapefruit around the time of a dose.

Timing with sexual activity

Take the tablet at least 30 minutes before anticipated sexual activity. So the effect can start as early as 30 minutes but builds further over the next 2 hours. In particular, the window extends up to 36 hours after a single dose, so dosing does not need to be as closely tied to sexual timing as it is with sildenafil.

Storage

  • Store below 30 degrees Celsius in a dry place.
  • Keep in the original blister pack.
  • Keep out of the sight and reach of children.
  • Do not use after the expiry date on the pack.

Treatment dosage tadalafil 20mg

The standard adult dose

The standard BNF and SmPC dose for adult men 18 and over is one 20mg tablet taken 30 minutes to 36 hours before sexual activity. So the timing is flexible within that window. In particular, maximum one tablet in any 24-hour period, regardless of whether the first dose worked. Furthermore, sexual stimulation is still needed for an erection to happen; the tablet does not act on its own.

When to start lower (10mg)

Some patients start at 10mg to see how they respond before moving to 20mg. So the dose can be stepped up or down based on response and side effects. In particular, this is a prescriber decision based on how significant the ED is and what medicines or health conditions you have.

The daily low-dose option (2.5mg or 5mg)

Tadalafil is also available at 2.5mg or 5mg once daily as an alternative to on-demand dosing. So this suits men who have sex frequently (more than twice a week), who want continuous coverage, or who also need treatment for benign prostatic hyperplasia (BPH) symptoms. However, the daily low-dose option is a separate product; the 20mg tablet is specifically for on-demand use and should not be taken daily.

Dose in kidney impairment

For creatinine clearance less than 30 mL/min, the maximum dose is 10mg on-demand (not 20mg). So the 20mg tablet is contraindicated in severe kidney impairment and end-stage kidney disease. In particular, make sure your prescriber knows about any kidney condition and has a recent renal function test.

Dose in liver impairment

Mild to moderate liver impairment: maximum 10mg on-demand (not 20mg). Severe liver impairment: tadalafil is contraindicated. So tell your prescriber about any known liver condition.

If a dose does not work

Do not take a second tablet in the same 24-hour period. However, if tadalafil consistently gives no response at 20mg over 4 to 8 attempts (with proper sexual stimulation), discuss with your prescriber. In particular, this may mean switching to another PDE5 inhibitor, looking at non-vascular causes, or adjusting other medicines you take.

When to stop and see your prescriber

Stop tadalafil and contact your prescriber urgently if:

  • You develop chest pain, pressure, or tightness during or after sex.
  • You develop sudden hearing loss, ringing in the ears, or dizziness.
  • You develop sudden visual loss or disturbance in one or both eyes.
  • You develop an erection lasting more than 4 hours (go to A&E).
  • You develop fainting or near-fainting.
  • You start a nitrate or riociguat for any other condition.

Warnings and precautions for tadalafil 20mg tablets

Absolute contraindication with nitrates

Any form of nitrate (GTN spray or tablets, isosorbide mononitrate, isosorbide dinitrate, amyl nitrite / "poppers") is an absolute contraindication with tadalafil. So the combination causes profound and prolonged hypotension that can be fatal. In particular, this includes recreational "poppers" that some men use socially. Furthermore, if emergency nitrates become clinically necessary, at least 48 hours must have passed since the last tadalafil dose.

Absolute contraindication with riociguat

Riociguat (used in pulmonary hypertension and chronic thromboembolic pulmonary hypertension) stimulates soluble guanylate cyclase. So combining it with a PDE5 inhibitor produces excessive hypotension. In particular, this is an absolute contraindication.

Cardiovascular safety

Sexual activity itself carries cardiovascular risk, and PDE5 inhibitors should be used with care in cardiovascular disease. So tadalafil 20mg is contraindicated in heart attack within the last 90 days, stroke within the last 6 months, unstable angina, angina during sex, severe heart failure (NYHA 3 to 4), severe hypertension or hypotension, and serious uncontrolled arrhythmia. In particular, a cardiovascular risk assessment is part of the consultation.

Alpha-blockers

Alpha-blockers (doxazosin, tamsulosin, alfuzosin) can cause symptomatic hypotension when combined with PDE5 inhibitors. So the combination is used with care, often with staggered timing or dose reduction. In particular, if you take an alpha-blocker for BPH or hypertension, tell your prescriber before starting tadalafil.

Vision and hearing

Rare but serious effects on vision (non-arteritic anterior ischaemic optic neuropathy, NAION) and on hearing (sudden sensorineural hearing loss) have been reported with PDE5 inhibitors. So any sudden visual loss, visual disturbance, hearing loss, ringing in the ears, or dizziness during or after a dose needs urgent review. In particular, a previous episode of NAION is a contraindication.

Priapism

Priapism is a prolonged erection lasting more than 4 hours that does not subside after sex. So this is a medical emergency because it can permanently damage erectile tissue. In particular, go to A&E immediately if an erection lasts more than 4 hours. Furthermore, patients at higher risk (sickle cell disease, multiple myeloma, leukaemia, anatomical penile deformity) need extra caution.

Kidney and liver impairment

Severe kidney impairment or dialysis: 20mg is contraindicated; a reduced dose (5mg or 10mg) may be used. Severe liver impairment: tadalafil is contraindicated. So tell your prescriber about any known kidney or liver condition.

Older men

Tadalafil is used across adult ages, though cardiovascular comorbidity rises with age. So in older men a careful cardiovascular check is particularly important before prescribing.

Not for women or children

Tadalafil 20mg is licensed only for adult men. So it is not studied or approved for women or anyone under 18.

Side effects of tadalafil 20mg tablets

Most men tolerate tadalafil well. However, some side effects are worth knowing about:

Common effects

  • Headache
  • Flushing (warmth or redness in the face or neck).
  • Indigestion (dyspepsia) or stomach discomfort.
  • Nasal congestion (stuffy nose).
  • Back pain.
  • Muscle aches (myalgia), especially in the thighs or lower back.
  • Pain in the limbs.

Managing common effects

  • Headache: usually mild; paracetamol or ibuprofen are safe.
  • Flushing: usually brief; a cool drink and a cool environment help.
  • Indigestion: usually mild; an over-the-counter antacid helps if needed.
  • Back pain or muscle aches: tadalafil-specific effect that usually settles over the first few doses; stop and discuss if persistent.
  • Nasal congestion: usually short-lived; standard decongestants (not combined with pseudoephedrine during angina) are an option.

Less common effects

  • Dizziness or lightheadedness.
  • Blurred vision or visual disturbance.
  • Palpitations
  • Hot flushes.
  • Chest pain (always needs review).
  • Transient hypotension.

Rare but important

  • Non-arteritic anterior ischaemic optic neuropathy (NAION), sudden loss or disturbance of vision.
  • Sudden sensorineural hearing loss or ringing in the ears (tinnitus).
  • Priapism (erection lasting more than 4 hours), A&E.
  • Myocardial infarction or stroke (rare, cardiovascular risk discussion applies).
  • Severe allergic reaction (facial swelling, breathing difficulty, widespread rash).
  • Seizures

When to stop and get help

Stop tadalafil and contact your prescriber or NHS 111 urgently if you develop sudden visual loss or disturbance, sudden hearing loss or tinnitus, chest pain, severe palpitations, fainting, facial swelling, or severe allergic reaction. So for priapism (an erection lasting more than 4 hours), go directly to A&E. Furthermore, for life-threatening symptoms such as chest pain or suspected stroke, call 999.

Yellow Card scheme: report any suspected side effects via https://yellowcard.mhra.gov.uk or the Yellow Card app. Reporting helps make medicines safer for everyone.

Drug interactions with tadalafil 20mg

Nitrates (absolute contraindication)

All nitrates (GTN spray, GTN tablets, isosorbide mononitrate, isosorbide dinitrate, amyl nitrite / "poppers") are absolutely contraindicated with tadalafil. So the combination causes profound, prolonged hypotension that can be fatal. In particular, this is the single most important interaction to understand, and includes recreational "poppers". Furthermore, if nitrates become clinically necessary, at least 48 hours must pass since the last tadalafil dose.

Riociguat (absolute contraindication)

Riociguat (used in pulmonary hypertension) stimulates soluble guanylate cyclase. So combining it with a PDE5 inhibitor causes excessive hypotension. In particular, this is an absolute contraindication.

Alpha-blockers

Alpha-blockers (doxazosin, tamsulosin, alfuzosin, prazosin) can cause symptomatic postural hypotension when combined with tadalafil. So the combination is used with care, often by staggering doses or starting at low dose. In particular, tamsulosin 0.4mg daily with tadalafil 20mg is generally better tolerated than doxazosin with tadalafil.

Strong CYP3A4 inhibitors

Ketoconazole, itraconazole, ritonavir, clarithromycin, and some HIV protease inhibitors are strong CYP3A4 inhibitors that raise tadalafil levels. So in patients on these drugs, lower doses of tadalafil (10mg maximum, with longer intervals) are usually appropriate.

CYP3A4 inducers

Rifampicin, carbamazepine, phenytoin, phenobarbital, and St John's Wort are CYP3A4 inducers that lower tadalafil levels and reduce its effect. So higher doses or alternative treatment may be needed.

Antihypertensive medicines

Tadalafil has a mild blood-pressure-lowering effect that can add to antihypertensives. However, this is usually clinically well tolerated. So no routine dose change is needed, but tell your prescriber about all blood pressure medicines you take.

Grapefruit and grapefruit juice

Grapefruit inhibits CYP3A4 in the gut wall and can raise tadalafil levels. So limit grapefruit and grapefruit juice around the time of a dose.

Alcohol

Moderate alcohol is generally compatible with tadalafil. However, heavy alcohol can impair erectile function in its own right and may add to the mild blood-pressure-lowering effect of tadalafil. So moderation is sensible.

Frequently asked questions about tadalafil 20mg tablets

How quickly does tadalafil 20mg work?

Tadalafil can start working within 30 minutes of taking a tablet, with full effect built over 2 hours. However, it works best with proper sexual stimulation; it does not create arousal on its own.

How long does tadalafil 20mg last?

A single 20mg tablet can have effect for up to 36 hours. So during that window, sexual stimulation is more reliably followed by a satisfactory erection. In particular, this does not mean a continuous 36-hour erection, and sexual stimulation is still needed each time.

What is the difference between tadalafil and Cialis?

They are the same medicine. So Cialis is the branded version made by Lilly, and tadalafil is the generic name. In particular, generic tadalafil has been available in the UK since 2017, when the Cialis patent expired, at much lower cost.

Can I take tadalafil 20mg every day?

No, the 20mg tablet is for on-demand use only, maximum one in any 24 hours. So if you want continuous coverage, the daily low-dose form (tadalafil 2.5mg or 5mg once daily) is the right product, not the 20mg tablet.

Can I take tadalafil with alcohol?

Moderate alcohol is generally fine with tadalafil. However, heavy alcohol can impair erectile function in its own right and may add to the mild blood-pressure-lowering effect of tadalafil. So moderation is sensible.

Can I take tadalafil if I take blood pressure medicines?

Usually yes, provided your blood pressure is well controlled. However, nitrates (GTN, isosorbide mononitrate) are an absolute contraindication, and alpha-blockers (doxazosin, tamsulosin) need careful combination. So tell your prescriber about every blood pressure medicine you take.

What should I do if I get an erection lasting more than 4 hours?

Go to A&E immediately. So a prolonged erection (priapism) can permanently damage erectile tissue if not treated within hours. In particular, do not wait to see if it will settle on its own.

Can tadalafil cause heart attack or stroke?

Tadalafil does not cause heart attack or stroke on its own in men with normal cardiovascular status. However, sexual activity itself has a small cardiovascular risk, and tadalafil is contraindicated after recent heart attack or stroke and in severe heart failure. So a cardiovascular check before starting matters.

Can I take tadalafil with sildenafil?

No. Do not combine two PDE5 inhibitors. So this does not increase effectiveness, it just doubles side-effect risk. In particular, if one PDE5 inhibitor has not worked, switch rather than stack.

What if tadalafil 20mg does not work?

If tadalafil 20mg does not give adequate response over 4 to 8 attempts with proper sexual stimulation, discuss with your prescriber. So this may mean switching to another PDE5 inhibitor, considering the daily low-dose option, or looking at non-vascular contributors (psychological, hormonal, relationship). In particular, baseline bloods (lipids, HbA1c, testosterone) are worth checking if not done recently.

Can tadalafil help with premature ejaculation?

Tadalafil is not licensed for premature ejaculation. So dapoxetine (Priligy) is the licensed on-demand treatment for premature ejaculation. However, some men who have both ED and premature ejaculation find that treating the ED helps the whole picture.

Can women take tadalafil?

No. Tadalafil is not licensed for women. So it is not studied or approved for female sexual dysfunction.

Does tadalafil increase libido?

No. Tadalafil treats the physiological erection pathway, not sexual desire. So it does not increase libido or arousal. In particular, low libido often reflects low testosterone, relationship factors, stress, or other medicines, and these need their own assessment.

Can I drive after taking tadalafil?

For most men, yes. However, some patients experience dizziness or visual disturbance, which can affect driving. So do not drive until you know how tadalafil affects you.

Can I take tadalafil with testosterone replacement therapy?

Yes, this combination is common and well tolerated. So tadalafil and testosterone work on different parts of the sexual response and can be complementary. In particular, if ED has not responded to tadalafil alone and testosterone is low, adding TRT often improves response.

How do I know if my ED is physical or psychological?

A clue is whether you still get morning erections or erections during masturbation. So men who get these but struggle during partnered sex often have a significant psychological component. In particular, men who have lost all spontaneous erections usually have a more vascular or hormonal picture, and this is where PDE5 inhibitors tend to work best.

Is tadalafil available over the counter?

Yes, some strengths of tadalafil are available over the counter.

Can I take tadalafil if I have diabetes?

Yes, diabetes is one of the common causes of ED, and tadalafil is effective in diabetic men (though response rates are slightly lower than in non-diabetic men). So good diabetes control also helps erectile function in its own right.

What if I accidentally take two tablets?

Contact NHS 111 or your prescriber. So this does not increase effectiveness, it just increases side-effect risk (headache, flushing, muscle aches, hypotension). In particular, watch for any symptoms of low blood pressure (dizziness, fainting) and avoid any nitrates in the next 48 hours.

What is tadalafil 20mg?

Tadalafil 20mg is a prescription-only tablet used to treat erectile dysfunction in adult men. So it is the full-strength on-demand dose of tadalafil, a phosphodiesterase type 5 (PDE5) inhibitor. In particular, tadalafil is the generic name for the drug originally sold as Cialis.

What is tadalafil 20mg used for?

Tadalafil 20mg is used for the on-demand treatment of erectile dysfunction in adult men. So one tablet is taken 30 minutes to 36 hours before anticipated sexual activity, maximum one tablet per 24 hours. In particular, the 20mg strength is not licensed for benign prostatic hyperplasia (BPH); the 5mg daily low-dose form is used for that.

How long does tadalafil 20mg take to work?

Tadalafil 20mg can start to work within 30 minutes, with full effect built over 2 hours. However, this is the time to a response with proper sexual stimulation; without stimulation, nothing happens. So take the tablet at least 30 minutes before anticipated sexual activity for the best result.

When to take tadalafil 20mg?

Take tadalafil 20mg 30 minutes to 36 hours before anticipated sexual activity. So the timing is flexible within that window. In particular, maximum one tablet in any 24-hour period, and sexual stimulation is still needed for an erection to happen.

How to use tadalafil 20mg?

Swallow one 20mg tablet whole with a glass of water, 30 minutes to 36 hours before anticipated sexual activity. So it can be taken with or without food. In particular, maximum one tablet in any 24-hour period, and sexual stimulation is still needed. Furthermore, do not combine tadalafil with any nitrate (GTN, isosorbide mononitrate, or recreational "poppers"), as this combination can cause fatal hypotension.

Where can I buy tadalafil 20mg?

Tadalafil 20mg is a prescription-only medicine (POM) in the UK, so it can only be dispensed by a UK-registered pharmacy against a valid prescription. So we supply tadalafil 20mg at Courier Pharmacy through our erectile dysfunction consultation pathway at courierpharmacy.co.uk. In particular, our Superintendent Pharmacist (or a partner prescriber) reviews each request before dispensing.

How many tadalafil 20mg can I take?

Maximum one 20mg tablet in any 24-hour period. So this is the licensed maximum on-demand dose. In particular, taking more does not improve response and increases side-effect risk (headache, flushing, muscle aches, hypotension). Furthermore, if 20mg does not give adequate response over several attempts, speak to your prescriber rather than taking a higher dose.

How often can you take tadalafil 20mg?

Up to one tablet in any 24-hour period, taken on demand. So the 20mg strength is for on-demand use, not for daily use. In particular, if you want continuous coverage or more than once-a-day use, the daily low-dose form (tadalafil 2.5mg or 5mg once daily) is a separate product and a better fit.

What happens if I take 20mg of tadalafil?

With sexual stimulation, 20mg of tadalafil improves blood flow to the penis and makes a satisfactory erection more likely for up to 36 hours. However, common side effects include headache, flushing, nasal congestion, mild indigestion, back pain, and muscle aches, which usually settle within a few hours. In particular, if nothing happens at all, remember that tadalafil is not an aphrodisiac; sexual stimulation is still needed.

What happens if I take two 20mg tadalafil tablets?

Taking two 20mg tablets does not improve response and significantly increases side-effect risk (severe headache, flushing, muscle aches, dizziness, hypotension). So if you have taken two tablets, contact NHS 111 for advice and avoid any nitrate (GTN, isosorbide mononitrate, "poppers") for at least 48 hours. In particular, go to A&E or call 999 if you develop chest pain, severe dizziness, fainting, or an erection lasting more than 4 hours.

Can I take 20mg tadalafil two days in a row?

You can take one 20mg tablet on consecutive days, provided you keep to the maximum of one tablet per 24-hour period. However, continuous daily use of the 20mg strength is not the licensed pattern. So if you anticipate wanting coverage on consecutive days regularly, discuss the daily low-dose form (tadalafil 2.5mg or 5mg once daily) with your prescriber. In particular, the daily low-dose option is designed for continuous use rather than on-demand use.

How long does 20mg tadalafil stay in your system?

Tadalafil has an elimination half-life of around 17.5 hours. So most of a 20mg dose is cleared from the body within 3 to 4 days. However, the clinically useful effect on erections lasts up to 36 hours after a single dose. In particular, do not take any nitrate within 48 hours of a tadalafil dose, as the interaction window extends beyond the useful-effect window.

How effective is tadalafil 20mg?

In the pivotal clinical trials, around 70 to 80 percent of men achieved successful intercourse on tadalafil 20mg, compared with about 30 to 40 percent on placebo. So it is one of the most effective pharmacological treatments for erectile dysfunction. However, response is individual, and tadalafil works best when there is a vascular (rather than purely psychological) basis to the ED. In particular, lifestyle factors (cardiovascular health, weight, smoking, alcohol, sleep) also affect how well it works.

Tadalafil 20mg how long does it last?

Tadalafil 20mg can have effect for up to 36 hours after a single dose. So this is the "weekend tablet" duration that distinguishes tadalafil from the shorter-acting PDE5 inhibitors. In particular, this does not mean a continuous 36-hour erection, it means that during this window sexual stimulation is more reliably followed by a satisfactory erection.

Tadalafil 20mg how long does it take to work?

Tadalafil 20mg can start to work within 30 minutes, with full effect built over 2 hours. So take the tablet at least 30 minutes before anticipated sexual activity. In particular, sexual stimulation is still needed; tadalafil amplifies the response, it does not create one.

Tadalafil 20mg what is it used for?

Tadalafil 20mg is used for the on-demand treatment of erectile dysfunction in adult men. So one tablet is taken 30 minutes to 36 hours before sexual activity, maximum one in 24 hours. In particular, the 20mg strength is not licensed for BPH; a separate 5mg daily low-dose form is used for that.

Tadalafil 20mg how long before it works?

Allow at least 30 minutes between taking a tadalafil 20mg tablet and anticipated sexual activity. So the effect builds further over the next 2 hours and continues up to 36 hours. In particular, there is no need to time the dose tightly to sex, which is one of tadalafil's practical advantages.

Tadalafil 20mg when to take?

Take tadalafil 20mg at least 30 minutes before anticipated sexual activity, up to 36 hours in advance. So the window is flexible. In particular, maximum one tablet per 24 hours, and sexual stimulation is still needed.

Where can I buy tadalafil 20mg?

Tadalafil 20mg is a UK prescription-only medicine, so it can only be dispensed by a UK-registered pharmacy against a valid prescription. So we supply tadalafil 20mg at Courier Pharmacy through our erectile dysfunction consultation at courierpharmacy.co.uk.

How long does 20mg of tadalafil last?

A 20mg dose of tadalafil can have effect for up to 36 hours. So this is substantially longer than sildenafil (4 to 6 hours) or vardenafil (4 to 5 hours). In particular, the long window is what gives tadalafil its "weekend tablet" nickname.

Tadalafil 20mg how long lasts?

Tadalafil 20mg lasts up to 36 hours after a single on-demand dose. So this long duration is one of its main selling points. However, this does not mean a 36-hour erection, it means the ability to achieve an erection with sexual stimulation stays improved during that window.

How long does 20mg tadalafil last?

A single 20mg tadalafil tablet lasts up to 36 hours. So the clinical effect extends well beyond the 17.5-hour elimination half-life of the drug itself. In particular, this is why one Friday evening dose can cover sexual activity on Saturday as well.

How long does 20mg tadalafil last? (quick answer)

Up to 36 hours. So this is the practical duration of useful effect from a single 20mg on-demand dose. In particular, no more than one tablet in any 24-hour period, even within that 36-hour window.

How long for 20mg tadalafil to work?

Allow at least 30 minutes between taking the tablet and sexual activity. So the full effect builds over 2 hours. In particular, the response depends on proper sexual stimulation; tadalafil does not create arousal on its own.

Tadalafil 20mg how long before sex?

Take tadalafil 20mg at least 30 minutes before anticipated sex. So the window of effect extends up to 36 hours. In particular, this means dosing does not need to be as tightly coupled to sexual timing as with sildenafil.

Tadalafil 20mg what is it?

Tadalafil 20mg is a prescription-only PDE5 inhibitor tablet used to treat erectile dysfunction in adult men. So it is the generic name for the drug originally marketed as Cialis. In particular, the 20mg strength is the full-strength on-demand dose.

How long does 20mg tadalafil take to work?

Around 30 minutes to a first response, with full effect built over 2 hours. So the start is slower than avanafil (15 to 30 minutes) but the overall window is much longer at up to 36 hours. In particular, taking it earlier rather than later gives the most reliable result.

How often can I take 20mg tadalafil?

Maximum one 20mg tablet in any 24-hour period, taken on demand. So the 20mg strength is not for daily use. In particular, if you want continuous coverage, the daily low-dose tadalafil 2.5mg or 5mg once daily is a separate product designed for that pattern.

Tadalafil 20mg how to use?

Swallow one 20mg tablet whole with water, 30 minutes to 36 hours before anticipated sexual activity. So it can be taken with or without food. In particular, maximum one per 24 hours, and sexual stimulation is still needed. Furthermore, do not combine with any nitrate (GTN, isosorbide mononitrate, "poppers"), as this combination can be fatal.

How long for tadalafil 20mg to work?

Allow at least 30 minutes before sex, with full effect reached by 2 hours. So the response depends on sexual stimulation during the active window. In particular, the window stays open for up to 36 hours after a single dose.

Tadalafil 20mg last how long?

A single on-demand 20mg tadalafil dose lasts up to 36 hours. So this is longer than any other oral PDE5 inhibitor used in the UK. In particular, the long duration suits men who prefer spontaneity rather than tight sex-dosing coupling.

How long does 20mg of tadalafil work?

A 20mg tadalafil dose remains clinically effective for up to 36 hours. So within that window, sexual stimulation is more likely to produce a satisfactory erection. In particular, the effect fades gradually over that period rather than cutting off sharply.

How fast does tadalafil 20mg work?

Tadalafil 20mg can start to work within 30 minutes of taking the tablet. However, avanafil (Spedra) is faster (15 to 30 minutes). So if speed of onset is the priority, avanafil may fit better; if duration is the priority, tadalafil wins.

How long does it take tadalafil 20mg to work?

Around 30 minutes to a first response, with full effect by 2 hours after the dose. So this is a slightly slower onset than sildenafil for some men but with a far longer window of action. In particular, the longer window often outweighs the slightly slower start for men who want flexibility.

How often can you take 20mg of tadalafil?

Up to one 20mg tablet in any 24-hour period, taken on demand. So the 20mg strength is on-demand only. In particular, if you want continuous coverage or sex on consecutive days regularly, the daily low-dose form (2.5mg or 5mg once daily) is a separate product and the right fit.

When to take tadalafil 20mg?

Take tadalafil 20mg at least 30 minutes before anticipated sexual activity, and up to 36 hours in advance. So the dosing window is wide. In particular, maximum one tablet per 24 hours, and sexual stimulation is still needed.

How long does 20mg of tadalafil take to work?

About 30 minutes to a first response, with full effect built by 2 hours. So allow at least 30 minutes between the tablet and sex. In particular, taking it on an empty stomach or a full stomach makes very little difference for tadalafil, unlike sildenafil.

How long does it take for 20mg tadalafil to work?

30 minutes to a first response and up to 2 hours to peak effect. So the full 36-hour window opens after the first 30 to 120 minutes. In particular, if the first attempt at sex happens within that window, the response is usually at its most reliable.

What happens if I take two 20mg tadalafil?

Taking two 20mg tablets does not improve the response and significantly increases side-effect risk (severe headache, flushing, muscle aches, dizziness, hypotension). So contact NHS 111 for advice and avoid any nitrate for at least 48 hours. In particular, go to A&E or call 999 if you develop chest pain, severe dizziness, fainting, or an erection lasting more than 4 hours.

Cialis tadalafil 20mg how to use?

Cialis is the brand name for tadalafil; the usage is identical. So swallow one 20mg Cialis tablet whole with water, 30 minutes to 36 hours before anticipated sexual activity. In particular, maximum one tablet per 24 hours, and never combine with nitrates or riociguat.

How long does 20mg of tadalafil work for? (quick answer)

Up to 36 hours per on-demand dose. So the clinical effect persists well after the drug levels have fallen. In particular, this is one of tadalafil's main differentiators from sildenafil and vardenafil.

How often can you take tadalafil 20mg?

Maximum one 20mg tablet in any 24-hour period. So this is the licensed on-demand pattern. In particular, continuous daily use at the 20mg strength is not the licensed pattern; use the daily low-dose form instead if continuous coverage is wanted.

How many 20mg tadalafil can I take?

One 20mg tablet per 24-hour period. So this is the maximum on-demand dose. In particular, taking more does not improve response and raises the risk of headache, muscle aches, hypotension, and (rarely) priapism or visual effects.

Tadalafil 20mg how often to take?

On demand, up to one 20mg tablet per 24-hour period. So the 20mg strength is not scheduled daily; it is used when sexual activity is anticipated. In particular, patients wanting continuous coverage move to the daily low-dose option (2.5mg or 5mg once daily).

How long does the effect of tadalafil 20mg last?

Up to 36 hours after a single on-demand dose. So this is longer than any other oral PDE5 inhibitor in current UK use. In particular, this long window is one of the main reasons patients and partners choose tadalafil.

How often can you take 20mg tadalafil?

Up to one 20mg tablet in any 24-hour period, taken on demand. So the 20mg strength is for on-demand use. In particular, continuous daily use at 20mg is not the licensed pattern; the daily low-dose form (2.5mg or 5mg once daily) is the right product for continuous coverage.

How long does it take for tadalafil 20mg to work?

Around 30 minutes to a first response, up to 2 hours to peak. So take the tablet at least 30 minutes before anticipated sex. In particular, the overall window stays open for up to 36 hours.

Where to buy tadalafil 20mg?

Tadalafil 20mg is prescription-only in the UK, so it can only be dispensed by a UK-registered pharmacy against a valid prescription. So Courier Pharmacy supplies tadalafil 20mg through our erectile dysfunction consultation at courierpharmacy.co.uk, with discreet UK delivery. In particular, if you already have a GP prescription, we can also dispense against that.

What are tadalafil 20mg tablets?

Tadalafil 20mg tablets are a prescription-only PDE5 inhibitor used to treat erectile dysfunction in adult men. So each film-coated tablet contains 20mg of tadalafil, the active ingredient also sold under the brand name Cialis. In particular, the 20mg strength is the full-strength on-demand dose.

How to take tadalafil 20mg?

Swallow one 20mg tablet whole with a glass of water, 30 minutes to 36 hours before anticipated sexual activity. So it can be taken with or without food. In particular, maximum one tablet per 24 hours, and sexual stimulation is still needed for an erection to happen.

ED as a cardiovascular signal

ED is often an early warning of cardiovascular disease, particularly in men under 60. So treating only the ED without checking cardiovascular risk is a missed opportunity. In particular, we routinely suggest baseline bloods (lipids, HbA1c, testosterone) for men with new ED who have not had these recently, and we flag when a GP conversation about statins or blood pressure makes sense.

Lifestyle changes that help ED

Several lifestyle changes have real evidence for improving erectile function: weight loss (particularly abdominal fat), regular aerobic exercise, stopping smoking, limiting alcohol, better sleep, and treating sleep apnoea. So medicines like tadalafil work better when the underlying vascular biology is being supported. In particular, improving these factors is also good for cardiovascular health as a whole.
