Eligibility, who can order Testosterone Cypionate 2000mg/10ml

This product is strictly for patients already stable on an existing TRT protocol. We do not start new TRT patients on this vial. To order, you must supply:

  • A valid, in-date private prescription from a UK-registered prescriber.

Why the blood pressure reading

Testosterone can raise blood pressure in some men, and uncontrolled hypertension is a risk factor during TRT. A recent reading lets your prescriber confirm that your blood pressure sits in a safe range before we dispense. Take two seated readings five minutes apart, after resting quietly for five minutes. What is Testosterone Cypionate 2000mg/10ml for?

Testosterone Cypionate 2000mg/10ml is prescribed for men with testosterone deficiency (hypogonadism). The medicine replaces the testosterone that the body does not make in adequate amounts. The BSSM 2024 UK Guidelines set the diagnostic threshold and clinical criteria.

Prescribers use it in the following patient groups:

  • Men with primary hypogonadism, such as Klinefelter syndrome or post-orchidectomy.
  • Men with secondary (central) hypogonadism, such as pituitary pathology or long-term opioid use.
  • Men with age-related or adult-onset testosterone deficiency confirmed on two early-morning fasted total testosterone readings below the BSSM threshold.
  • Men already stable on another testosterone ester who want to switch to cypionate for route flexibility or steady-state kinetics.

What this product is NOT for:

  • Performance enhancement in men with normal testosterone.
  • Body composition goals outside a medical TRT indication.
  • Women (except in rare specialist indications outside the scope of this listing).

How does Testosterone Cypionate work?

Testosterone cypionate is a long-acting prodrug of testosterone. The 8-carbon cypionate ester attaches to the testosterone molecule and slows its release.

After an injection, three steps follow:

  • Step 1, the oil depot sits in the muscle or subcutaneous fat.
  • Step 2, enzymes slowly cleave the cypionate ester to release free testosterone into the blood.
  • Step 3, testosterone binds to androgen receptors in target tissues.

Target tissues include muscle, bone, skin, brain, and reproductive organs. The hormone supports:

  • Lean body mass and muscle strength.
  • Bone mineral density.
  • Libido and erectile function.
  • Mood and energy.
  • Red blood cell production.
  • Secondary sexual traits.

A small share of testosterone converts to dihydrotestosterone (DHT) by 5-alpha-reductase. Another share converts to oestradiol by aromatase. Both metabolites shape the overall clinical effect.

The cypionate ester has a half-life of around 7 to 8 days. That half-life allows a steady testosterone level with weekly or more frequent dosing. Subcutaneous injection delivers the ester into the fat layer, where absorption kinetics match or slightly smooth those of deep IM injection. SC therefore gives lower peak-to-trough swings, which is the rationale for SC microdosing schedules.

Dosing options and schedules

Your prescriber will choose a schedule based on your bloods, symptoms and preferences. Modern UK TRT practice offers several options:

Intramuscular (IM) schedules

  • Once a week IM, 100mg (0.5ml) into deep gluteal or vastus lateralis muscle.
  • Twice a week IM, 50mg (0.25ml) on two days spaced evenly across the week.
  • Every two weeks IM, 200mg (1.0ml), less common now because of wider peak-to-trough swings.

Subcutaneous (SC) microdosing schedules

SC dosing splits the weekly dose into smaller, more frequent injections. Many men find this smooths out mood, energy and libido across the week.

  • Three times a week SC, 0.16ml (32mg) on three evenly spaced days, delivers around 96 to 100mg per week.
  • Every other day SC, around 0.14ml (28mg), delivers around 98mg per week.
  • Daily SC, around 0.07 to 0.08ml (14 to 16mg), delivers around 100mg per week.

How long does one 10ml vial last?

A single 10ml vial holds 2000mg of cypionate. Vial life depends on your weekly dose:

  • 100mg per week, around 20 weeks (5 months).
  • 125mg per week, around 16 weeks (4 months).
  • 150mg per week, around 13 weeks (3 months).

Wipe the vial septum with an alcohol swab before every draw. Track the first-opening date on the label. Discard the vial after the beyond-use-date, usually 28 days after first puncture.

How to inject safely

Pick the right syringe for the route. Oil-based testosterone is thicker than water-based medicines, which affects your choice of needle.
  • For IM injection, use a Unifix 1ml Luer Lock syringe. Draw with a wider needle (21G x 1.5 inch) and swap to a finer injection needle (23G or 25G x 1 inch).
  • For SC microdosing, you have two sensible options. Option one, a Luer Lock syringe (Unifix 1ml) with a 21G draw needle and a 25G to 27G x 0.5 inch injection needle. Option two, a Unifine or similar fixed-needle 29G or 30G 1ml insulin syringe, which draws and injects through the same fine needle. The draw is slower through a 29G or 30G, but many men prefer the simpler one-syringe workflow for daily or alternate-day microdoses, particularly when the cypionate is in a thinner MCT oil base.
  • IM technique: inject at a 90-degree angle into deep muscle (ventrogluteal or vastus lateralis), aspirate briefly, deliver slowly over 10 to 20 seconds.
  • SC technique: pinch a fold of fat on the abdomen, outer thigh or flank, inject at a 45 to 90-degree angle, deliver slowly.
  • Rotate sites each dose to avoid lipohypertrophy.
  • Drop used syringes into a yellow sharps bin, no recapping.

Missed dose

For a once-weekly schedule, take the missed dose as soon as you remember within 2 to 3 days. Otherwise skip it and resume your regular schedule. For SC microdoses given multiple times a week, skip the missed dose and continue as normal. Do not double up.

Vial life and storage

  • At 100mg per week, the 10ml vial lasts around 20 weeks (5 months).
  • At 125mg per week, around 16 weeks (4 months).
  • At 150mg per week, around 13 weeks (3 months).
  • Store at room temperature below 25 degrees Celsius, upright, out of direct sunlight. Do not refrigerate.
  • Track the first-opening date on the label. Discard after the beyond-use-date, usually 28 days from first puncture.
  • Keep out of sight and reach of children and non-prescribed adults.

Monitoring on treatment, following BSSM 2024

BSSM 2024 UK guidelines set out a structured monitoring schedule:

  • Baseline, total testosterone (fasted, 7am to 11am), free testosterone or SHBG, LH, FSH, FBC with haematocrit, HbA1c, lipids, PSA, LFTs, U&E, plus a validated symptom score.
  • Three months, repeat testosterone (timed to injection cycle, usually mid-cycle or trough), FBC, PSA and symptom score.
  • Six months, as above plus lipids and HbA1c.
  • Annual review, full baseline panel plus PSA and digital rectal examination where indicated.

Your prescriber will adjust the dose against mid-cycle or trough levels and your symptoms. The target range is usually mid-to-upper normal for the laboratory used. A haematocrit above 0.54 usually triggers a dose reduction, phlebotomy or a switch to a lower-peak schedule.

Warnings and precautions

Do not use (contraindications)

Do not use Testosterone Cypionate if any of the following apply:

  • A known or suspected prostate cancer.
  • A known or suspected breast cancer in men.
  • A known allergy to testosterone cypionate or any excipient (benzyl alcohol, benzyl benzoate, the oil carrier).
  • Severe liver disease.
  • Nephrotic syndrome.
  • Pregnancy or breastfeeding (the product is for men only).

Speak to your prescriber before use

Several situations need prescriber review before you continue cypionate:

  • A raised PSA, a prostate nodule, or a strong family history of prostate cancer.
  • A baseline haematocrit above 0.50.
  • Untreated obstructive sleep apnoea.
  • Severe heart failure or recent myocardial infarction.
  • High blood pressure above 160/100 mmHg.
  • Plans to father children in the next 6 to 12 months.
  • A bleeding disorder or anticoagulant therapy.
  • A history of venous thromboembolism (DVT or PE).
  • Current use of another androgen or anabolic preparation.
  • Diabetes or impaired glucose tolerance (testosterone affects insulin sensitivity).

Precautions during use

Keep the following safety points in mind during long-term treatment:

  • Attend every scheduled monitoring blood test. Dose adjustments depend on your bloods.
  • Report new or worsening urinary symptoms promptly, as these may signal prostate changes.
  • Report swelling or pain in the calf, breathlessness or chest pain immediately, these may signal a blood clot.
  • Report mood changes, including hypomania or persistent irritability.
  • Monitor your blood pressure at home and report any sustained rise.
  • Report unusual bleeding or bruising, especially if you also take an anticoagulant.
  • Avoid sharing needles, vials or syringes, both for infection control and for Controlled Drug compliance.

Legal classification and controlled drug status

In the United Kingdom, testosterone cypionate is:
  • A prescription-only medicine (POM) under the Human Medicines Regulations 2012.
  • A Schedule 4 Part 2 Controlled Drug (CD Anab, anabolic steroid class) under the Misuse of Drugs Regulations 2001.
  • Not subject to the Schedule 4 Part 1 (CD Benz) safe-custody and record-keeping requirements. It is still a controlled drug for possession and supply.
Possession of testosterone cypionate without a prescription is a criminal offence under the Misuse of Drugs Act 1971. Personal importation without a valid UK prescription is also an offence. Courier Pharmacy supplies this product only against a valid private prescription from a UK-registered prescriber. Cypionate is not a UK-licensed branded product. We supply it either as an unlicensed imported medicine (under the MHRA Specials import framework) or as a UK-compounded medicine. The dispensing label states the source and the beyond-use-date.

Pregnancy, breastfeeding and fertility

Testosterone Cypionate is not suitable in pregnancy or breastfeeding. It virilises a female fetus and passes into breast milk. We prescribe it for men with testosterone deficiency only. Men planning to father children should be aware of fertility effects. Testosterone replacement suppresses the hypothalamic-pituitary-gonadal axis. It reduces or stops sperm production. Options to protect fertility include:
  • Semen cryopreservation before starting therapy.
  • Specialist regimens using HCG with or without hMG under endocrinologist supervision.
Discuss fertility plans at your baseline consultation.

Storage and shelf life

Store Testosterone Cypionate 2000mg/10ml at room temperature below 25 degrees Celsius. Keep the vial upright and out of direct sunlight. Do not refrigerate, as cold oil becomes very viscous and hard to draw. Protect the vial from freezing. Keep the vial in its original carton until first use. Write the first-opening date on the label. Discard the vial after the beyond-use-date, usually 28 days from first puncture. A brief return to room temperature should clear any mild cloudiness. Discard any vial with persistent cloudiness, discolouration or visible particles. Keep out of sight and reach of children. Store the sharps bin, needles and vial together in a locked or secured location.

Sharps disposal

Used needles and syringes are sharps waste under the UK Health and Safety (Sharp Instruments in Healthcare) Regulations 2013. Simple rules to follow:
  • Drop the used syringe into a UK-standard yellow sharps bin without recapping.
  • Fill the bin no more than three-quarters full.
  • Seal the lid and arrange collection through your local authority sharps-collection service.
  • Never place sharps in household bins or recycling.
When a vial reaches the end of its beyond-use-date, drop the vial into the sharps bin as well. It has held a Controlled Drug and should not go in general or recycling waste. Return any unused vials to the pharmacy for destruction under Controlled Drug procedures.

Compatible syringes and needles

Oil-based testosterone is too thick for fine fixed-needle insulin syringes. The standard set-up is a Luer Lock syringe with a wider draw needle and a finer injection needle.
  • Unisharp or Unifix 1ml Luer Lock syringe, the standard choice for weekly or microdosed injection.
  • BD Microlance 21G x 1.5 inch green needle, for drawing from the vial.
  • BD Microlance 23G x 1 inch blue needle, for IM injection in most adults.
  • BD Microlance 25G x 1 inch orange needle, for IM injection in a leaner adult or SC in a slimmer body.
  • BD Microlance 27G x 0.5 inch grey needle, for SC microdosed injection where volume is less than 0.3ml.
  • Yellow sharps bin, 1-litre or 2.5-litre, UK-standard BS 7320.

Reporting adverse reactions

Side effects

Common and expected effects of adequate replacement include improved libido, erectile function, mood, energy and lean body mass. However, cypionate can also cause side effects. The frequencies below follow standard SmPC convention.

Very common and common side effects

  • Erythrocytosis (raised haematocrit and haemoglobin), the most common dose-related effect.
  • Acne or oily skin, especially during initial dose titration.
  • Fluid retention and mild oedema.
  • Injection-site reactions, mild redness, bruising or discomfort.
  • Increased libido.
  • Hair thinning or male-pattern hair loss acceleration in susceptible men.

Uncommon side effects

  • Gynaecomastia (breast tissue growth) through peripheral conversion to oestradiol.
  • Mood changes, including irritability or hypomania at supraphysiological levels.
  • Suppression of sperm production and testicular atrophy.
  • Headache.
  • Weight gain.
  • Rise in liver function tests.

Rare but serious side effects, seek urgent review

  • Venous thromboembolism (DVT or PE), signs include a swollen or painful calf, breathlessness or chest pain.
  • Worsening of untreated sleep apnoea.
  • Faster growth of pre-existing (not new) prostate cancer.
  • Severe allergic reaction, rash, swelling of face or throat, breathing difficulty, call 999.
  • Severe mood change, aggression or persistent hypomania.
  • New or worsening urinary symptoms suggesting prostate changes.

Drug interactions

Testosterone can interact with several classes of medicine. Tell your prescriber and pharmacist about every medicine, supplement and over-the-counter product you take.

Interactions that may need dose adjustment

  • Anticoagulants (warfarin, DOACs), testosterone can increase bleeding risk. Warfarin needs INR monitoring, and your anticoagulant clinic should know you have started TRT.
  • Insulin and oral hypoglycaemics (metformin, sulphonylureas, gliptins), testosterone improves insulin sensitivity. Your diabetes team may need to lower your dose to prevent hypoglycaemia.
  • Corticosteroids (prednisolone, dexamethasone), both drugs cause fluid retention. The combination may worsen oedema.
  • ACTH, same fluid-retention risk as corticosteroids.
  • Cyclosporine, testosterone can raise cyclosporine levels, which may increase toxicity.
  • Thyroxine, testosterone affects thyroid-binding globulin, which may alter free thyroxine levels in some patients.

Interactions to avoid

  • Other androgens or anabolic steroids, do not stack testosterone products. Combined use increases erythrocytosis, cardiovascular risk and side effects.
  • Erythropoiesis-stimulating agents (epoetin, darbepoetin), combined use markedly increases the risk of erythrocytosis and thrombosis.
  • Oestrogens, antagonise the effect of testosterone. Discuss with your prescriber if these are needed for another indication.

Interactions with lifestyle and supplements

  • Alcohol, moderate alcohol is not a contraindication. Heavy or chronic alcohol affects liver function, mood, sleep and lipids. Keep within UK Chief Medical Officers' guidance of up to 14 units a week.
  • Grapefruit juice, no significant interaction with injectable testosterone.
  • Herbal supplements (St John's Wort, ashwagandha, tongkat ali), tell your prescriber. Some herbs claim testosterone effects and may alter your bloods.
  • 5-alpha-reductase inhibitors (finasteride, dutasteride), these reduce DHT conversion. They do not stop testosterone treatment but may affect prostate symptoms and hair-related effects.
  • Aromatase inhibitors (anastrozole, letrozole), only used off-label on prescriber advice for symptomatic high oestradiol.

Frequently asked questions

Am I eligible to order this product?

Only if you are already stable on an existing TRT protocol. You must upload a valid private prescription, bloods within the last 3 months, and a blood pressure reading within the last 14 days. New TRT patients should start with our full TRT consultation pathway.

How recent does my blood pressure reading need to be?

Within the last 14 days. Take two seated readings five minutes apart after resting quietly for five minutes. Upload the lower of the two as a photo or screenshot. If your reading is above 160/100 mmHg, do not place the order. Speak to your GP or TRT prescriber first.

How many mg per ml is this product?

Testosterone Cypionate 2000mg/10ml is a 200mg/ml preparation. Each 1ml draws 200mg of testosterone cypionate.

How long does the 10ml vial last?

At the standard TRT dose of 100mg per week, around 20 weeks or 5 months. At 150mg per week, around 13 weeks or 3 months.

Is Testosterone Cypionate licensed in the UK?

No. We supply it as an unlicensed imported medicine under the MHRA Specials import framework, or as a UK-compounded medicine from a licensed Specials manufacturer. The UK-licensed long-acting testosterone esters are Sustanon, Testosterone Enantate and Nebido.

Can I inject Testosterone Cypionate subcutaneously?

Yes. Studies show SC administration achieves comparable or smoother testosterone levels than IM injection. Peak-to-trough variation is lower. Many men prefer SC because the needles are smaller and the technique is simpler.

What is a typical SC microdose?

For a target of 100mg per week, a common schedule is 0.16ml (32mg) three times a week on evenly spaced days. Another option is daily SC at around 0.07 to 0.08ml (14 to 16mg). Your prescriber chooses the schedule based on your bloods and symptoms.

What is the difference between Testosterone Cypionate and Testosterone Enantate?

Both are long-acting single testosterone esters with similar half-lives (around 7 to 8 days). Both are clinically equal at matched doses. Cypionate is the US standard preparation. Enantate is UK-licensed as Testosterone Enantate.

What syringe and needle should I use?

A 1ml Luer Lock syringe (such as the Unifix 1ml Luer Lock) with a wider draw needle (21G x 1.5 inch) and a finer injection needle (23G or 25G for IM, 25G to 27G for SC). Fixed-needle 29G or 30G insulin syringes are not suitable.

Does Testosterone Cypionate affect fertility?

Yes. Testosterone replacement suppresses the hypothalamic-pituitary-gonadal axis. It reduces or stops sperm production. If you plan to father children, discuss sperm banking or alternative regimens (HCG monotherapy, HCG plus hMG) with your prescriber first.

What monitoring will I need?

BSSM 2024 UK guidelines recommend baseline, 3-month, 6-month and annual bloods. The panel covers total testosterone, free testosterone or SHBG, FBC with haematocrit, PSA, lipids and HbA1c, plus a symptom score. Your prescriber books these in with you.

Is Testosterone Cypionate a controlled drug?

Yes. In the UK, testosterone cypionate is a Schedule 4 Part 2 Controlled Drug (CD Anab) under the Misuse of Drugs Regulations 2001. Safe-custody and CD-register rules do not apply. Possession without a prescription is still a criminal offence.

Can I bring Testosterone Cypionate from the US into the UK?

Personal importation without a valid UK prescription is a criminal offence under the Misuse of Drugs Act 1971. If you are moving to the UK on prescribed cypionate, bring your original prescription and medical records. Arrange a UK consultation to transfer care.

How should I store the vial?

Room temperature, below 25 degrees Celsius, upright, out of direct sunlight. Do not refrigerate. Track the first-opening date and discard by the beyond-use-date on the label.

What happens if I miss a dose?

For a once-weekly schedule, take the missed dose as soon as you remember within 2 to 3 days. Otherwise skip it and resume your regular schedule. For SC microdoses given multiple times a week, skip the missed dose and continue as normal. Do not double up.

Will Testosterone Cypionate cause gynaecomastia?

It can, through peripheral conversion of testosterone to oestradiol by aromatase. Risk is dose-related and varies between men. Your prescriber monitors for breast tissue changes and measures oestradiol if needed. An aromatase inhibitor is only considered in symptomatic cases.

Can I drink alcohol on Testosterone Cypionate?

Moderate alcohol is not a contraindication. However, heavy or chronic alcohol affects liver function, mood, sleep and lipids. All of these are tracked on TRT. Keep within UK Chief Medical Officers' low-risk guidance of up to 14 units a week.
