Description

Ramipril 5mg Capsules, UK-licensed ACE inhibitor for blood pressure, heart, and kidney protection

Ramipril 5mg Capsules are a UK-licensed ACE inhibitor. In short, they lower blood pressure and protect the heart and kidneys. First, 5mg is a common maintenance dose. However, your prescriber may titrate up or down based on your response.

The active works through the renin-angiotensin system. Specifically, ramipril blocks the enzyme that makes angiotensin II. As a result, blood vessels relax, pressure drops, and the heart’s workload falls. In addition, kidney filtration pressure eases.

Ramipril is used for several conditions. Firstly, it treats high blood pressure in adults. In addition, it is first-line in NICE guidelines for under-55s who are not of African or Caribbean family background. Also, it is used in heart failure and after a heart attack.

The MHRA flags several key signals. First, pregnancy is a hard contraindication (teratogenic in all trimesters). Second, dry persistent cough affects around 1 in 10 users. Third, rare angioedema can be serious. Finally, acute kidney injury risk rises with NSAIDs and diuretics together.

Once-daily ACE inhibitor for blood pressure and heart protection

Ramipril 5mg is a once-daily maintenance dose. Here is how we supply it:

  • First, a prescriber reviews your blood pressure, kidney function, and other medicines.
  • Next, a 28-capsule pack covers 28 days of once-daily use.
  • Then, review at 4 weeks for response and side effects.
  • Also, blood tests at baseline and 1-2 weeks after dose changes are standard.

What are Ramipril 5mg capsules?

Ramipril is a prodrug. First, the liver converts it to its active form, ramiprilat. Then ramiprilat blocks angiotensin-converting enzyme (ACE). As a result, less angiotensin II is formed.

In addition, less aldosterone is released. Therefore, blood vessels relax, salt and water retention eases, and blood pressure falls. Also, pressure inside the kidney filters drops. This is why ramipril protects kidney function in diabetes.

Where Ramipril 5mg Capsules sits among UK options

  • Ramipril (this product, POM), prototype ACE inhibitor, generic.

How Ramipril 5mg Capsules work

How ramipril blocks ACE

Ramipril is a prodrug. First, the liver converts it to ramiprilat, the active form. Then ramiprilat binds angiotensin-converting enzyme (ACE), also called kininase II.

ACE does two main jobs. First, it converts angiotensin I to angiotensin II, a potent vasoconstrictor. In addition, it breaks down bradykinin, a vasodilator. As a result, blocking ACE lowers angiotensin II and raises bradykinin.

Ramiprilat binds ACE with high affinity. Therefore, its blood pressure effect lasts around 24 hours from a single daily dose.

Why lower angiotensin II means lower blood pressure

Angiotensin II has several roles. First, it constricts blood vessels. In addition, it triggers aldosterone release from the adrenal glands.

Aldosterone makes the kidneys retain sodium and water. As a result, blood volume rises and pressure climbs. Therefore, less angiotensin II means less constriction and less fluid retention.

Also, ramipril reduces efferent arteriolar tone in the kidney. Specifically, pressure inside the glomerulus drops. This is why ramipril protects kidney function in diabetes and other renal disease.

The bradykinin side (cough and angioedema)

ACE normally breaks down bradykinin. First, when ACE is blocked, bradykinin accumulates. In addition, substance P levels rise. As a result, airway sensory nerves get irritated.

Therefore, around 10 to 20 percent of patients develop a dry persistent cough. It typically starts in weeks to months after starting. However, it is not dangerous. If troublesome, your prescriber may switch to an ARB like losartan or candesartan.

Rarely (0.1 to 0.5 percent), bradykinin accumulation causes angioedema, swelling of lips, tongue, or airway. This is a medical emergency. First-generation and Afro-Caribbean patients are at higher risk.

Dosing and titration logic

The 5mg capsule is a mid-range maintenance dose. First, treatment usually starts at 1.25mg or 2.5mg once daily. Then the dose doubles every 2 to 4 weeks based on response and tolerance.

The 5mg dose covers most hypertensive patients. In addition, it is a common target dose in heart failure. However, the maximum licensed dose is 10mg once daily. Therefore, your prescriber may go higher if BP remains above target.

In plain terms

Your body has a system that controls blood pressure. In short, a hormone called angiotensin II tells blood vessels to tighten and the kidneys to hold on to salt and water.

Ramipril blocks the enzyme that makes angiotensin II. As a result, blood vessels relax and the kidneys release more fluid. Then blood pressure drops.

In addition, ramipril eases pressure inside the kidney filters. Therefore, it also protects kidneys in diabetes and other kidney disease.

There is one known quirk. First, around 1 in 10 users develop a dry persistent cough. However, the cough is not dangerous. If it bothers you, your prescriber can switch you to a similar drug called an ARB (losartan or candesartan) that does not cause the cough.

Finally, give it time. Blood pressure effect starts within hours. However, the full effect builds over 2 to 4 weeks.

Frequently asked questions about Ramipril 5mg capsules

What is ramipril?

Ramipril is a UK-licensed ACE inhibitor. In short, it is one of the most widely prescribed blood pressure medicines in the UK. First, it is a prodrug activated in the liver. In addition, it is also used in heart failure, after a heart attack, and for kidney protection.

What is ramipril used for?

Ramipril treats several conditions. Firstly, high blood pressure (hypertension) is the most common use. In addition, it is used in heart failure, after a heart attack, in diabetic and non-diabetic kidney disease, and to lower cardiovascular risk in patients with existing heart disease.

What is ramipril for?

Ramipril is prescribed for high blood pressure, heart failure, heart protection after a heart attack, and kidney protection. First, it relaxes blood vessels. In addition, it eases strain on the heart and kidneys. Therefore, it treats a cluster of related cardiovascular problems.

What are ramipril tablets for?

Ramipril tablets (and capsules) are for blood pressure control and heart and kidney protection. In particular, they are used in hypertension, heart failure, post-heart-attack care, and nephropathy. Dose depends on the condition and your response, usually once a day.

What does ramipril do?

Ramipril blocks the enzyme that makes angiotensin II, a hormone that tightens blood vessels. As a result, blood vessels relax and blood pressure falls. In addition, kidney filtration pressure eases. Therefore, the heart works less hard and the kidneys are protected.

How does ramipril work?

Ramipril is a prodrug activated in the liver to ramiprilat. First, ramiprilat blocks angiotensin-converting enzyme (ACE). Then angiotensin II levels drop and bradykinin levels rise. As a result, blood vessels dilate, blood pressure falls, and kidney filtration pressure eases.

Is ramipril a beta blocker?

No, ramipril is not a beta blocker. First, ramipril is an ACE inhibitor. However, beta blockers (bisoprolol, atenolol, propranolol) work differently, by blocking adrenaline receptors on the heart. Also, the two drug classes are often used together in heart failure or after a heart attack.

Is ramipril a blood thinner?

No, ramipril is not a blood thinner. First, it does not affect blood clotting. However, it does lower blood pressure. True blood thinners are drugs like warfarin, apixaban, rivaroxaban, or aspirin. Therefore, you may be prescribed both a blood thinner and ramipril together.

Is ramipril an ACE inhibitor?

Yes, ramipril is an ACE inhibitor. In fact, it is one of the prototype ACE inhibitors used worldwide. Other ACE inhibitors include lisinopril, perindopril, and enalapril. All of them end in “-pril” by naming convention.

Is ramipril a diuretic?

No, ramipril is not a diuretic. First, diuretics (water tablets) work directly on the kidney to increase urine output. However, ramipril works on blood vessels and the renin-angiotensin system. Diuretics include bendroflumethiazide, indapamide, and furosemide. Ramipril and a diuretic are often used together.

Is ramipril a statin?

No, ramipril is not a statin. Statins (atorvastatin, simvastatin, rosuvastatin) lower cholesterol. However, ramipril lowers blood pressure and protects the heart and kidneys. The two are often prescribed together in patients with cardiovascular disease. They do different jobs.

How long does ramipril take to work?

The blood-pressure-lowering effect starts within 1 to 2 hours of the first dose. However, peak effect at a given dose builds over 2 to 4 weeks. Therefore, full response is assessed at 4 weeks. For heart or kidney protection, the benefits accrue over months to years.

How long does it take for ramipril to work?

Ramipril begins to lower blood pressure within 1 to 2 hours. In addition, maximal effect at any given dose builds across the first 2 to 4 weeks of treatment. Therefore, your prescriber usually reviews BP at 4 weeks before deciding whether to adjust the dose.

How quickly does ramipril work?

Ramipril acts quickly in one sense, blood pressure falls within hours of the first dose. However, the full therapeutic effect at a given dose takes 2 to 4 weeks. In short, first-dose fall is often modest, with the real benefit building over weeks.

How long for ramipril to work?

Allow 4 weeks at any given dose for full blood pressure effect. First, there is a response within hours. However, the plateau takes longer. Therefore, do not expect your home BP readings to drop dramatically on day 1. Steady adherence matters more than instant effect.

How long does ramipril stay in your system?

Ramipril itself has a short half-life, around 1 hour. However, its active form ramiprilat has an effective half-life of 13 to 17 hours, with some persistent ACE binding beyond that. Therefore, once-daily dosing is enough. Full clearance from the body takes around 4 to 5 days after the last dose.

When to take ramipril?

Once a day, at a consistent time. First, many patients take ramipril in the morning. However, evening dosing may reduce first-dose dizziness and improve overnight BP control. Therefore, choose a time you will stick to. Take with or without food.

Is 1.25 mg of ramipril a low dose?

Yes, 1.25mg is the lowest standard adult starting dose of ramipril. First, it is used for first-dose titration, especially in heart failure, elderly patients, or those on diuretics. However, 1.25mg alone rarely controls blood pressure long-term. Therefore, most patients titrate upwards from here.

Is 1.25 mg of ramipril a high dose?

No, 1.25mg is a starting dose. In fact, it is the lowest standard adult dose available. The maximum licensed dose is 10mg once daily. Therefore, 1.25mg sits at the bottom of the range, often used only for the first 1 to 2 weeks before titration.

How much does 2.5 mg ramipril lower blood pressure?

On average, 2.5mg once daily lowers systolic BP by around 5 to 10 mmHg and diastolic BP by 3 to 7 mmHg. However, response varies widely. First-dose fall can be larger in volume-depleted or elderly patients. Therefore, home BP monitoring at 1 to 2 weeks helps judge response.

Is 5mg of ramipril a high dose?

No, 5mg is a mid-range maintenance dose. First, treatment usually starts at 1.25mg or 2.5mg once daily. Then the dose doubles every 2 to 4 weeks. In addition, 5mg controls many hypertensive patients. The maximum is 10mg once daily.

How much does 5 mg ramipril lower blood pressure?

On average, 5mg once daily lowers systolic BP by around 10 to 15 mmHg and diastolic BP by 5 to 10 mmHg. However, individual response varies. Therefore, home BP readings and a 4-week clinic review are the best guide to whether 5mg is enough for you.

Is ramipril 10 mg a high dose?

Yes, 10mg is the maximum licensed daily dose of ramipril. First, it is used when 5mg has not achieved BP target. In addition, 10mg is the target dose in cardiovascular risk reduction trials (HOPE study). However, side effects become more common at the top of the range.

Is 20 mg ramipril too much?

Yes. The maximum licensed dose of ramipril is 10mg once daily. Therefore, 20mg is above the licensed range. If BP is uncontrolled on 10mg, your prescriber will usually add a second class (a calcium channel blocker, a diuretic, or an ARB), not raise ramipril further.

What is the highest dose of ramipril?

The highest licensed dose is 10mg once daily. First, this is used in resistant hypertension, cardiovascular risk reduction, and some heart failure patients. However, if BP is not controlled on 10mg, the usual next step is to add a second drug, not to go higher on ramipril.

What is the maximum dose of ramipril per day?

10mg once daily is the maximum licensed adult dose. In short, this is the ceiling. If 10mg does not control blood pressure, your prescriber will add a different class of medicine. Common additions include amlodipine (calcium channel blocker) or indapamide (thiazide-like diuretic).

Can ramipril cause a cough?

Yes, dry persistent cough is a well-known side effect of all ACE inhibitors. First, around 10 to 20 percent of users develop it. In addition, it is caused by bradykinin build-up in the airways. However, it is not dangerous. If troublesome, your prescriber can switch you to an ARB (losartan, candesartan).

Is ramipril cough dangerous?

The cough itself is not dangerous. First, it is dry, non-productive, and tickly rather than chesty. However, it can be very disruptive to sleep and daily life. In addition, it does not improve with cough syrups. Therefore, if it bothers you, your prescriber can switch to an ARB that does not cause the cough.

How to get rid of ramipril cough?

The only reliable way is to stop ramipril and switch to a non-ACE class. First, your prescriber usually switches you to an ARB like losartan, candesartan, or irbesartan. In addition, the cough typically settles within 1 to 4 weeks of stopping. Cough suppressants rarely help while on ramipril.

Does ramipril make you tired?

Fatigue is a recognised side effect of ramipril. First, it is more common in the first few weeks or after a dose increase. In addition, low blood pressure can cause tiredness. However, if fatigue is severe or persists past 4 weeks, speak to your prescriber about dose or BP review.

Does ramipril cause weight gain?

No, ramipril is not associated with weight gain. In fact, if anything, mild fluid loss early in treatment may cause a small weight drop. However, if you notice sudden weight gain with swelling of ankles or breathlessness, this could signal fluid retention or heart failure, review promptly.

Does ramipril cause erectile dysfunction?

Yes, uncommonly. First, erectile dysfunction is listed as an uncommon side effect of ACE inhibitors. However, high blood pressure itself is a common cause of ED. Therefore, the medicine may not be the only driver. If you develop new ED on ramipril, speak to your prescriber about options.

Does ramipril cause diarrhea?

Yes, uncommonly. First, nausea, diarrhoea, and abdominal discomfort are listed common-to-uncommon side effects. In addition, they usually settle within the first 2 to 4 weeks. However, if diarrhoea is severe or persists, speak to your prescriber. Dehydration can raise your risk of acute kidney injury on ramipril.

Does ramipril make you pee more?

Ramipril is not a diuretic, so it does not directly make you pee more. However, early in treatment some patients notice a modest increase in urine output as fluid shifts. In addition, if you are on a diuretic alongside, that drug drives the urination. Therefore, timing of ramipril (morning or evening) can be adjusted.

What are the side effects of ramipril?

Common side effects include dry persistent cough, headache, dizziness, fatigue, raised potassium, and modest raised creatinine. In addition, uncommon effects include rash, taste disturbance, and erectile dysfunction. Rarely, angioedema (lip, tongue, or airway swelling) occurs, which is a medical emergency.

What are the most common side effects of ramipril?

Dry persistent cough is the most common (around 1 in 10). In addition, headache, dizziness (especially first-dose), fatigue, and mild dizziness on standing are frequent. Also, blood tests may show modest rises in potassium and creatinine in the first few weeks. Most settle or are tolerable.

What are the long-term side effects of ramipril?

Long-term, ramipril is generally well tolerated. First, the cough and dizziness tend to settle or persist at a steady level. However, kidney function and potassium should be monitored annually. In addition, rare long-term risks include chronic hyperkalaemia and, very rarely, late-onset angioedema.

What tablets should not be taken with ramipril?

Avoid or review these: regular NSAIDs (ibuprofen, naproxen, diclofenac), potassium-sparing diuretics (spironolactone, amiloride), potassium supplements, aliskiren (in diabetes), lithium (needs monitoring), and other ACE inhibitors or ARBs together. Always tell your prescriber or pharmacist what you are taking.

What painkillers can I take with ramipril?

Paracetamol is the safest first choice with ramipril. First, it does not affect blood pressure or kidney function. However, avoid regular ibuprofen, naproxen, or diclofenac. In addition, for stronger pain, codeine-containing products (co-codamol) are usually safe short-term. For chronic pain, discuss options with your prescriber.

Can I take paracetamol and ramipril together?

Yes, paracetamol is safe to take with ramipril. First, it does not interact with ramipril or affect blood pressure. In addition, it does not stress the kidneys the way NSAIDs do. Therefore, paracetamol is the usual first-line painkiller for anyone taking ramipril.

Can you take ibuprofen with ramipril?

Occasional short-term ibuprofen (1 to 3 days) is usually fine. However, regular daily ibuprofen with ramipril raises the risk of acute kidney injury and reduces blood pressure control. Therefore, avoid regular NSAID use. Use paracetamol first. If you need regular pain relief, speak to your prescriber.

Can I take magnesium with ramipril?

Yes, standard magnesium supplements are usually safe with ramipril. First, magnesium does not interact with the ACE inhibition pathway. However, watch for additive BP-lowering if you are on high-dose magnesium. In addition, if your kidney function is reduced, ask your prescriber before starting any supplement.

Can I take antihistamine with ramipril?

Yes, standard antihistamines (loratadine, cetirizine, fexofenadine) are safe with ramipril. First, they do not interact pharmacologically. However, sedating antihistamines (chlorphenamine, promethazine) may add to any dizziness from ramipril. Therefore, non-sedating antihistamines are the better choice.

Disclaimer and extra notes

Pregnancy is a hard no.

Ramipril is contraindicated in all trimesters of pregnancy. First, it causes fetal kidney damage, lung underdevelopment, and skull defects. Therefore, if you plan a pregnancy or fall pregnant on ramipril, stop immediately and speak to your prescriber. Use effective contraception if childbearing potential.

The dry cough is common but not dangerous.

Around 1 in 10 users develop a dry persistent cough on ramipril. First, it reflects bradykinin build-up. However, it is not harmful. In addition, if it disrupts sleep or daily life, your prescriber can switch you to an ARB (losartan, candesartan) which does not cause the cough.

Angioedema is rare but a medical emergency.

Rarely, ACE inhibitors cause swelling of lips, tongue, or airway. First, this can appear hours or years after starting. In addition, it is more common in Afro-Caribbean patients. Therefore, if you develop facial, lip, or tongue swelling, stop ramipril and seek urgent medical help, call 999 if airway affected.

Triple whammy warning.

Combining ramipril with a diuretic and an NSAID (ibuprofen, naproxen, diclofenac) can cause acute kidney injury. First, each drug alone has modest renal effects. However, together they can tip the kidneys into failure, especially if dehydrated. Therefore, avoid regular NSAID use on ramipril plus a diuretic.

This information is for UK patients. In short, it does not replace a consultation with your prescriber. For medical emergencies, call 999. For urgent but non-emergency advice, call NHS 111. For routine queries, contact Courier Pharmacy (Rapid Medical Solutions Ltd, GPhC 9010051, Derby) through courierpharmacy.co.uk.
